Hospice RN Case Manager with Athena Hospice of Rhode Island

As a part-time Hospice RN Case Manager, you will play a crucial role in enabling patients and their loved ones to live each day as fully and comfortably as possible.

Job Overview:

We are seeking a dedicated and compassionate RN Case Manager to join our team. As a Hospice RN Case Manager, you will be responsible for planning, organizing, and directing the hospice care of assigned patients under the direction of the clinical director. This is an on-the-road position with mileage reimbursement, and you will be required to service the entire state of Rhode Island.

Responsibilities:

  • Conduct initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs
  • Provide professional nursing care by utilizing all elements of the nursing process
  • Develop and implement individualized care plans that establish goals and incorporate palliative nursing actions
  • Supervise hospice aides and provide guidance on patient care
  • Collaborate with patients, families, and healthcare professionals to develop a comprehensive plan of care
  • Provide emotional and spiritual support to patients and families

Requirements:

  • Must possess an active and unencumbered RN license in Rhode Island
