Job description

Step into a pivotal role supporting patients across all inpatient units within a leading Level 1 academic medical center. This contract opportunity focuses on acute inpatient case management and complex discharge planning for diverse, high-acuity patient populations. You will work collaboratively with an established team of RN Case Managers and an extensive Social Work department to ensure exceptional transitions of care in a dynamic setting.
Schedule:

  • Monday through Friday, 8:00 a.m. to 4:30 p.m.
  • No weekend shifts
  • Holiday coverage rotated as needed
Key Qualifications:
  • Active RN license with a minimum of 2 years' acute inpatient medical case management experience, including at least 1 year (within the past 3) in a large (500+ beds), busy Level 1 trauma or academic hospital ED
  • Significant recent acute care discharge planning for patients with complex medical diagnoses (within the last 2 years)
  • Prior travel experience in a Level 1 facility within the past 3 years
  • Proficient in EPIC charting and strong computer skills
  • Ability to adapt quickly with minimal unit training and contribute efficiently to a fast-paced environment
  • Flu vaccination required (medical exemptions must be documented by a healthcare provider)
  • Fingerprinting required prior to start (remote or in-person process within specified timeline)
  • Submission must detail trauma level, bed size, and specific duties for jobs held over the past 3 years
Role Responsibilities:
  • Manage a daily caseload of 15-20 patients, covering all inpatient units
  • Partner closely with Social Work, Respiratory Therapy, and Pharmacy as needed for effective discharge planning
  • Facilitate coordination of care and placement in contracted local facilities for qualified patients
  • Attend a two-week department-specific orientation and general facility onboarding
  • Uphold high professional standards in communication, collaboration, and documentation
