Public Health Nurse (PHN) - Federal Contract
Location: Phoenix, AZ
Job Type: Full-Time Contract (13-26 Weeks, with Option to Extend)
Department: Public Health Services
Reports To: Nurse Manager / Public Health Supervisor
Job Overview:
We are seeking dedicated and community-driven Public Health Nurses (PHNs) to support a federal healthcare contract in Phoenix, AZ. This role focuses on promoting population health, disease prevention, and community education. PHNs will work closely with interdisciplinary teams to deliver care, support public health initiatives, and improve health outcomes across diverse populations.
This is a great opportunity for nurses who are passionate about preventative care, community outreach, and making a broader impact beyond traditional clinical settings.
Key Responsibilities:
Conduct community health assessments and screenings
Provide education on disease prevention, wellness, and healthy lifestyle practices
Administer immunizations and support public health vaccination programs
Monitor and report communicable diseases in accordance with public health guidelines
Collaborate with healthcare providers, social services, and community organizations
Develop and implement care plans for individuals and families
Participate in outreach programs and public health initiatives
Maintain accurate documentation and reporting per federal and facility standards
Advocate for patient and community health needs
Ensure compliance with infection control and public health protocols
Minimum Qualifications:
Active Registered Nurse (RN) license (any state)
Bachelor of Science in Nursing (BSN) preferred
Current BLS certification (American Heart Association preferred)
At least 1 year of nursing experience (public health or community health preferred)
Preferred Qualifications:
Experience in public health, community health, or outpatient settings
Familiarity with federal or government healthcare systems
Strong background in patient education and preventive care
Experience with immunization programs or communicable disease tracking
