• Conduct comprehensive assessments of clients to develop personalized treatment plans aligned with their unique needs.
• Communicate effectively with supervisors, direct care staff, and family members about patient progress and treatment strategies.
• Provide supervisors with a schedule of appointments, home visits, and progress monitoring activities.
• Coordinate and lead home visits to conduct in-home assessments, chart reviews, and staff interviews.
• Ensure effective implementation of treatment plans and monitor patient outcomes.
• Participate actively in Interdisciplinary Team Meetings, sharing insights and strategies that enhance client care.
• Document and complete monthly and quarterly progress notes in a timely manner to maintain clear and organized client records.
• Conduct training programs to certify staff on the implementation of treatment plans and documentation practices.
• Perform follow-up interviews and observations, providing continuous support for clients' ongoing needs.
• Serve as a liaison to the public and mental health community, fostering relationships that support developmental objectives within the organization.
• Assist in the design and development of training materials that improve program delivery for individuals.

As an LPC at Congruent Health Center, you will work with a diverse population that includes both adults and youth. Your role will involve addressing various mental health issues such as anxiety, depression, trauma, and substance misuse. You will apply evidence-based therapeutic techniques tailored to individual circumstances, working collaboratively to empower clients to achieve their mental health goals.

• A Master's or Doctoral degree in Counseling, Psychology, or a closely related field.
• Possession of an active and unrestricted LPC license in the state of Colorado.
• A minimum of 2 years of counseling experience, particularly in a clinical setting.
• Demonstrated understanding of ethical and legal standards in mental health practice.
• Strong interpersonal and communication skills, with the ability to build rapport with clients from varied backgrounds.
• Proven collaboration skills within a healthcare team atmosphere.
• Familiarity with Electronic Medical Records (EMR) software and related technologies is essential.

• Specialization in family therapy, couples therapy, trauma-informed care, or substance misuse approaches.
• Experience working with diverse populations and a strong commitment to cultural competence in counseling.

The compensation for this role ranges from USD 90,000 to 100,000 annually, reflective of your experience and caseload. In addition to competitive salary, we offer:
