Key Responsibilities
  • Conduct comprehensive assessments of clients to develop personalized treatment plans aligned with their unique needs.
  • Communicate effectively with supervisors, direct care staff, and family members about patient progress and treatment strategies.
  • Provide supervisors with a schedule of appointments, home visits, and progress monitoring activities.
  • Coordinate and lead home visits to conduct in-home assessments, chart reviews, and staff interviews.
  • Ensure effective implementation of treatment plans and monitor patient outcomes.
  • Participate actively in Interdisciplinary Team Meetings, sharing insights and strategies that enhance client care.
  • Document and complete monthly and quarterly progress notes in a timely manner to maintain clear and organized client records.
  • Conduct training programs to certify staff on the implementation of treatment plans and documentation practices.
  • Perform follow-up interviews and observations, providing continuous support for clients' ongoing needs.
  • Serve as a liaison to the public and mental health community, fostering relationships that support developmental objectives within the organization.
  • Assist in the design and development of training materials that improve program delivery for individuals.
Patient Population / Client Focus
