Brandywine Counseling and Community Services is seeking a Full-Time Community Wellness Advocate for our Milford location!

BCCS is a community organization that prides itself on helping members of our community recover with dignity. We enhance the quality of life by creatively providing Education, Advocacy, Prevention, Early Intervention, and Treatment Services. We promote hope and empowerment to persons and families touched by mental illness, substance abuse, HIV and multiple occurring diagnoses, and their related challenges.


The Community Wellness Advocate has primary responsibility for providing quality, sufficient and appropriate prevention, advocacy, and treatment services in the community


What You’ll Do as a Community Wellness Advocate:

  • Conduct community-based HIV outreach, education, and prevention activities in high-priority communities
  • Provide HIV Counseling, Testing, and Referral (CTR) services in accordance with DPH and CDC guidelines
  • Deliver culturally responsive HIV prevention education, risk-reduction counseling, and health promotion services
  • Educate individuals on HIV transmission, STI prevention, PrEP awareness, safer sex practices, and available community resources
  • Facilitate linkage and referrals to HIV medical care, PrEP services, supportive services, and other community-based resources as appropriate
  • Assist participants in navigating healthcare and social service systems to improve access to care and support services
  • Build rapport and engage community members in nonjudgmental, trauma-informed, participant-centered interactions
  • Distribute prevention and risk reduction materials including condoms, lubricant, educational materials, and other approved supplies
  • Conduct individualized risk screenings and provide education based on participant needs and identified risk factors
  • Maintain accurate documentation, reporting, and data entry in required systems in accordance with program and DPH standards
  • Participate in community events, outreach initiatives, and partnership-building activities to increase awareness and engagement
  • Protect participant confidentiality and uphold practices that honor dignity, autonomy, diversity, and cultural humility
Requirements:

Knowledge, Skills, and Abilities:

  • Outreach, Engagement, and Community Health Worker practices as they relate to best practices and BCCS activities.
  • Outreach and prevention requirements, including Federal and State regulations, as well as contract requirements.
  • Behavioral health principles, standards, public health, wellness principles, advocacy principles and best practices.
  • Knowledge of local community resources
  • Data entry

Qualifications:

  • REQUIRED: High School Diploma or GED
  • REQUIRED: 2-years' Experience in the Human Services Field
  • PREFERRED: HIV Prevention Counseling, Testing, and Referral Training, CHW Certification
  • REQUIRED: Valid Driver’s License w/ 2 Points or Less

Schedule:

  • Full-Time Position
  • Monday – Friday
  • 7:00 a.m. - 3:00 p.m.

Pay:

  • Starting at $17/hour
  • Commensuration based on experience, education, and certification!
