Job description

Legacy Concierge is seeking a dedicated and compassionate Private Duty Registered Nurse to deliver exceptional 1:1 care to patients in the comfort of their homes. This role is designed specifically for RNs who are able to work independently, using your clinical skills to assess, triage, and apply preventive care techniques while having the support of your supervisor. Your focus will be delivering handsโ€‘on concierge care and fostering continuity and trust with your assigned patient during your shift.

Nurses in this role complete plan-of-care tasks under clinical supervision and are expected to complete structured onboarding, demonstrate patient-centered care, and build competence in the private home environment. All key responsibilities are contingent upon the patient's individualized plan of care.

Key Responsibilities Clinical Care & Patient Management
  • Deliver 8-12 hours, 1:1 skilled nursing care in the patientโ€™s home.
  • Perform comprehensive head-to-toe assessments including, but not limited to: evaluation of physical attributes, neurological status, respiratory function, cardiovascular status, skin integrity, and musculoskeletal function.
  • Monitor, administer, and document vital signs throughout the shift, including blood pressure, pulse, respiration, temperature, and oxygen saturation.
  • Manage and administer medications safely and accurately, including insulin management, blood pressure medications, oxygen, and other prescribed treatments, in accordance with the patient's plan of care.
  • Support activities of daily living (ADLs) and instrumental activities of daily living (IADLs) including, but not limited to: toileting, dressing, feeding, peri care, meal preparation, transportation, and companion care.
  • Perform G/J-tube feeding and medication administration as ordered.
  • Provide condition-specific management as directed by the plan of care.
  • Perform basic and advanced wound care and dressing changes.
  • Tracheostomy and ventilator care.
  • Seizure monitoring, oxygen therapy, oral suctioning, or other chronic condition management.
Communication & Documentation
  • Complete daily clinical notes accurately and in a timely manner using the agency's document systems (QAPI Plus, WellSky, RN Pad, etc.).
  • Communicate promptly and consistently with the RN Care Ambassador when there is a change in patient status, an incident, or a clinical concern requiring escalation.
  • Maintain confidentiality of all patients and company information at all times, in full compliance with HIPAA, infection control standards, and internal protocols.
  • Report incidents or clinical changes promptly to the RN Care Ambassador.
  • Provide emotional and physical support to the patient and their family throughout the shift.
  • Support case management and patient intake responsibilities as needed and as directed by the RN Care Ambassador.
Team & Operational Responsibilities

Required Qualifications
  • Active California Registered Nurse (RN) License in good standing
  • Graduate of an accredited BSN nursing program
  • Minimum 5 years of hospital experience, in-home concierge nursing experience, or skilled nursing experience
  • Current BLS/CPR certification
  • Demonstrated ability to work competently in a 1:1 private care environment.
  • Ability to work independently and follow instructions and ask questions when clarification is needed.
  • Strong communication, documentation, professionalism, and customer service skills.
Physical Requirements
  • Ability to stand, sit, kneel, and move for extended periods of time.
  • Ability to lift, transfer, or reposition patients with appropriate assistive techniques.
  • Ability to lift or carry equipment and supplies up to 50 pounds.
  • Use of clinical and technology equipment including tablets, phones, and navigating through EMR platforms.
  • Visual acuity to observe and assess patient conditions throughout the shift.
