Hospice Community Liaison/Marketer - Tyler Area.

Concho Hearts Hospice is growing, and weโ€™re looking for an experienced, relationship-driven Hospice Marketer to join our team in the Tyler area.

We are locally owned and operated, allowing us to respond quickly to patient needs and truly support the communities we serve.
We are seeking someone who not only understands hospice marketing but also believes in the mission and wants to make a meaningful impact.
About the Role:
As a Hospice Community Liaison, you will be a key leader in growing our presence in the San Antonio community. You will build trusted partnerships, educate providers and families, and guide patients through the transition to hospice care with compassion and professionalism.
Youโ€™ll have the support of an experienced leadership team, responsive clinical staff, and a company culture that values relationships over quotas.

What Youโ€™ll Do:
  • Build and maintain strong referral relationships with hospitals, facilities, physicians, and community partners.
  • Serve as a trusted resource for hospice education and support.
  • Guide patients and families through the referral and admission process.
  • Meet with families to explain hospice services with empathy and clarity.
  • Obtain consents for care respectfully and compassionately.
  • Lead educational in-services and community outreach events.
  • Help families understand what to expect from hospice care.
  • Partner closely with our clinical team to ensure smooth transitions from facility to home.
  • Support the continued growth of Concho Hearts Hospice in the community.
What Weโ€™re Looking For:
  • Minimum of 2+ years of hospice marketing experience (required).
  • Proven ability to build and maintain referral relationships.
  • Strong communication and presentation skills.
  • Self-motivated, dependable, and organized.
  • A compassionate heart for serving patients and families.
