Hospice Community Liaison/Marketer
Location
: Katy, TX, and surrounding area
Are you an experienced hospice marketer who thrives on building strong referral relationships and making a difference for patients and families?

 At Concho Hearts Hospice, our liaisons are trusted partners in the healthcare community. We’re looking for a motivated professional who is passionate about hospice care and enjoys developing meaningful partnerships with providers throughout the Houston area. 

What You’ll Do:
  • Develop and maintain referral relationships with hospitals, physicians, assisted living communities, and skilled nursing facilities.
  • Grow referral sources throughout the Houston service area.
  • Provide hospice education to healthcare providers and community partners.
  • Meet with patients and families to explain hospice services and obtain consents.
  • Coordinate closely with the clinical team to ensure seamless admissions.
  • Represent Concho Hearts Hospice as a trusted community resource.

What We’re Looking For:
  • Minimum 2 years of hospice marketing experience required.
  • Proven relationship-building and networking ability.
  • Self-motivated with strong organizational skills.
  • Comfortable presenting hospice education to providers and community groups.
  • Ability to work closely with clinical teams to coordinate patient admissions.
