Position Overview

Vision Works Youth Counseling Center is seeking a dedicated Licensed Clinical Social Worker (LCSW) to join our team in providing comprehensive mental health support for seniors from a remote setting. This full-time role offers the flexibility of working from home while delivering essential services to our clients at a hospice center, where your expertise will make a significant impact on their emotional and psychosocial well-being.

Work Environment

This primarily remote, full-time position allows for a flexible work schedule, with core hours from Monday to Friday, 8:00 AM to 4:30 PM. However, occasional evening and weekend hours may be required based on client needs, necessitating an adaptable approach to scheduling. On-call availability is essential for addressing urgent client situations. Working closely with an interdisciplinary team, you will have access to resources and support to ensure the highest quality of care.

Key Responsibilities
  • Conduct comprehensive assessments of the psychosocial needs of senior clients, taking into account their emotional, mental, and social aspects.
  • Collaborate with clients and their families to develop individualized care plans that align with their specific goals and preferences.
  • Provide counseling and emotional support to seniors and their families, helping them navigate transitions and significant life events.
  • Advocate for clients’ rights and help them access necessary community resources to enhance their quality of life.
  • Work alongside a team of healthcare professionals, including nurses and caregivers, to ensure coordinated and holistic client care.
  • Regularly visit clients to assess their well-being, living conditions, and to address any arising concerns.
  • Facilitate communication among family members regarding care and foster conflict resolution when necessary.
  • Educate clients and families about available community support groups and related resources to foster social engagement.
  • Maintain thorough and accurate documentation of client assessments, care plans, and interactions in compliance with regulatory standards.
  • Stay current with best practices in social work and ethical guidelines to ensure high standards of care.
Patient Population / Client Focus

The primary population served includes seniors, many of whom may be dealing with end-of-life issues, mental health challenges, or significant life changes. Your role will focus on providing empathetic support and effective interventions that address both the emotional and practical challenges faced by seniors and their families.

Qualifications
  • A Master’s degree in Social Work from an accredited institution is required.
  • Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
  • At least two years of experience in social work, preferably in a community or geriatric setting.
