• Conducting comprehensive mental health assessments to identify the impact of co-occurring medical conditions on patients' mental health and well-being.
• Developing and implementing individualized treatment plans that address both psychiatric issues and physical health concerns, ensuring coordinated care between mental health and medical teams.
• Facilitating individual and group therapy sessions utilizing evidence-based therapeutic modalities tailored to address the specific needs of patients.
• Collaborating with medical professionals to coordinate care effectively, enhancing treatment outcomes and patient satisfaction.
• Providing crisis intervention for patients experiencing acute mental health emergencies, ensuring their safety and appropriate care during critical times.
• Educating patients on the connections between mental and physical health, empowering them to take an active role in managing their conditions.
• Maintaining accurate and confidential patient records in compliance with legal and ethical standards, utilizing electronic health record systems.
• Actively participating in team meetings and case conferences, contributing insights and sharing best practices to foster a culture of continuous improvement.
• Engaging in professional development opportunities, including staying abreast of the latest research and best practices in integrated care.

The primary patient population for this role includes individuals with complex mental health needs exacerbated by co-occurring medical conditions. As a Licensed Mental Health Therapist at Maxi Wellness Corporation, you will work with patients dealing with conditions such as chronic pain, autoimmune disorders, and substance use disorders. Your expertise will be crucial in helping them navigate the intertwining challenges of physical and mental health.

• A Master's or Doctoral degree in Social Work, Mental Health Counseling, Marriage and Family Therapy, or Psychology from an accredited institution.
• Current and valid licensure as an LCSW, LPCC, or LMFT within the state of California.
• Proven clinical experience working with individuals who have co-occurring medical conditions, demonstrating strong assessment and intervention skills.
• Exceptional interpersonal and communication abilities, with the capacity to build rapport and trust with patients.
• Strong adherence to ethical guidelines and professional boundaries while maintaining patient confidentiality.
• Proficiency in using electronic health record systems and various computer applications relevant to mental health care.
• A collaborative mindset, with the ability to function effectively as part of an interdisciplinary care team.

• Experience working in outpatient office settings is a plus.
• Familiarity with integrated care models and practices.
• Previous involvement in crisis intervention and emergency response within mental health contexts.
• Demonstrated ability to engage in substantial case management activities.
