Hackensack Health System is seeking a dedicated Geriatric Mental Health Counselor to join our remote team. In this full-time position, you will play a vital role in providing specialized mental health care to elderly patients. This remote position allows for flexible working conditions ideally suited for mental health professionals seeking balance while continuing to provide quality care. As a member of our mental health team, you will engage in a dynamic work environment that values coordinated care, continuous training, and professional growth. The work schedule is Monday through Friday, from 9:00 AM to 5:00 PM, with occasional evening shifts to address the needs of our patients.

• Conduct thorough mental health assessments tailored to the unique needs of older adults, considering various psychosocial factors affecting their mental well-being.
• Develop and execute individualized treatment plans that integrate cognitive, social, and physical health considerations for each patient.
• Provide evidence-based therapeutic interventions, including individual therapy and family counseling, to effectively address a range of geriatric psychiatric conditions.
• Collaborate with a multidisciplinary team of geriatric psychiatrists, social workers, and healthcare providers to ensure holistic patient care.
• Offer crisis intervention support during mental health emergencies, acting promptly to stabilize and support elderly patients in distress.
• Educate patients and their families on mental health conditions, available treatment options, and community resources to promote informed decision-making.
• Maintain precise and confidential documentation to comply with legal and ethical standards, utilizing electronic health record (EHR) systems proficiently.
• Participate actively in team meetings and case consultations, contributing insights for the benefit of patient care.
• Stay updated on the latest research and best practices in geriatric psychiatry to continuously enhance service delivery.

Our focus is on the geriatric population, addressing complex mental health issues often faced by elderly individuals, including but not limited to cognitive impairments, mood disorders, and anxiety-related challenges. You will have the opportunity to work closely with patients experiencing significant life transitions, providing them the tools and support they need during these delicate phases of their lives.

• A Master’s or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution is required.
• Must hold a current and valid licensure as an LCSW, LPCC, or LMFT within the state of California.
• A minimum of two years of post-licensure experience working with the elderly population is preferred.
• Strong understanding of geriatric mental health disorders and evidence-based treatment modalities is essential.
• Demonstrated experience delivering individual and family therapy services to older adults, with a patient-centered approach.
• Excellent interpersonal and communication skills, fostering trust and rapport with patients and their families.
• Proficiency in electronic health record (EHR) systems and other relevant software applications.
• A collaborative spirit, capable of functioning effectively in a multidisciplinary team environment.

• Experience in a community health clinic setting, particularly with geriatric populations.
• Familiarity with crisis intervention techniques and immediate response strategies for elderly patients.
• Knowledge of community resources available for elderly patients and their families.

The position offers an attractive salary range from USD 115,000 to 130,000 annually, commensurate with experience.
