Job description

Job Summary
Provides ongoing monitoring of key quality metrics and outcomes data. Develops and implements quarterly training for providers and staff as it relates to changes or updates to quality metrics. Provides ongoing support to providers and staff based on provider-specific data and outcomes. Utilizes Epic and other designated software to streamline reporting and benchmarking. Supports quality initiatives and develops routine communication on quality metrics and outcomes data throughout the Shannon system. Utilizes quality metric data available to identify areas of opportunity to close care gaps and develop processes for improvement.
