Description
Our RN Case Managers are responsible for coordinating and managing the care of many of our high-risk patients, such as those with multiple chronic conditions. Duties include meeting with patients to manage their medical conditions, providing education, developing a care plan, appropriate follow-up and coordinating care with other clinical providers. As an essential member of our integrative care team, you will collaborate with multi-specialty colleagues to ensure that our patients are provided the highest quality of care.
Position Qualifications
  • RN License in the state of New Jersey
  • BSN preferred
  • 2 years of nursing experience, preferably in Case Management
  • BLS certification
  • Excellent interpersonal and computer skills
