Job description

General Summary: Responsible for the accurate and timely administration of medications, treatments, documentation, care coordination, supervision of nursing assistance and medication aids, and emergency response during assigned shifts.

Essential Job Duties:

  1. Directs the day-to-day functions of the nursing assistants and medication aides in accordance with rules, regulations, and guidelines that govern the long-term care facility.
  2. Ensure staffing sheet and assignment are completed for the shift.
  3. Participate in shift-to-shift reports with the team.
  4. Ensure controlled substances are counted and without discrepancies in accordance with procedures.
  5. Ensure nursing personnel comply with nursing procedures in the facility.
  6. Participates in facility surveys (inspections) made by authorized government agencies as requested.
  7. Admit, transfer,r and discharge residents as required.
  8. Conduct nursing assessment(s) with resident and initiate responsive action.
  9. Conduct resident medication reconciliation.
  10. Assist in planning the nursing services portion of the resident’s discharge plan.
  11. Complete accident and incident reports, as necessary.
  12. Complete and forward resident notifications to the appropriate departments as needed.
  13. Maintain daily census.
  14. Contact MD/NP/RP in response to resident needs or change of condition.
  15. Receive and process telephone and verbal orders form prescribers.
  16. Transcribe prescriber’s orders to resident order sheets, MAR, TAR, care plans, etc. as appropriate.
  17. Provide leadership to nursing personnel on your unit/shift.
  18. Make daily rounds ensuring nursing personnel are performing work assignments in accordance with standards of care. Report concerns to supervisor.
  19. Administers prescribed medications and treatments inclusive of but not limited to dressings, catheterization, suctioning, oxygen, etc.
  20. Process laboratory findings and reports in accordance with standards of care.
  21. Consult with prescribers in response to resident presentations, treatment, rehabilitation, etc.
  22. Provide direct resident care as needed.
  23. Obtain and process prescribed lab specimens as ordered.
  24. Coordinate prescribed diagnostics for the resident.
  25. Inform families and other contacts of the death of a resident.
  26. Ensures compliance with Infection Control standards.
  27. Maintain accurate and complete 24-hour reports.
  28. Ensure an adequate supply of PPE is readily available for personnel use.
  29. Ensure the unit/resident rooms are maintained in a clean, safe, and sanitary manner.

  30. Ensures nursing assistants and medication aides follow resident care plans and care cards, ensuring timely documentation.
  31. Interacts with residents, families, and visitors to solve problems maintaining goodwill which promotes positive customer relations.
  32. Maintains the self-respect, personal dignity, and physical safety of each patient
  33. Assures continuity of services and care
  34. Attends in-service training and education sessions, as assigned; completes Relias training on time every month.
  35. Must be familiar with emergency procedures and may be required to assist with evacuations.

Other Duties:

  1. May require flexible hours (weekends, evenings, and holidays).
  2. Must have reliable transportation.
  3. Able to lift and/or carry objects more than 25lbs.
  4. Utilize outstanding phone skills.
  5. Utilize excellent computer skills. Should be able to effectively use Microsoft Office Suite, MatrixCare, and all proprietary and user-end applications required for the successful completion of position duties
  6. Operate as the member of a team both within the department and across departments.
  7. Works beyond normal working hours on weekends and holidays and other shifts, as necessary.
  8. Subject to call-back during emergency conditions.
  9. Considered essential personnel during emergencies such as, but not limited to, pandemics.
  10. Perform other duties as assigned.
