Job description

The Case Manager supports Residents within the EAC unit by coordinating care across the interdisciplinary team, ensuring medical necessity, facilitating transitions, connecting Residents to community-based services. This role is essential to the EAC program's philosophy of recovery-oriented, trauma informed care, community integration and step-down readiness. The Case Manager ensures that Residents have access to appropriate supports during their stay and upon discharge.
QUALIFICATIONS:
  • BA DEGREE IN SOCIAL WORK OR RELATED FIELD REQUIRED.
  • Minimum of 2 years in Behavioral Health, preferably inpatient or residential care
  • Experience with complex case management needs Strong organizational, communication, and interdisciplinary collaboration skills.
  • Experience in working with community resources and payor groups.
