Job description

Contract RN Case Manager

Contract RN case manager based in Great Falls, MT for a 13-week assignment. The role includes housing and competitive pay, with the opportunity to coordinate patient care plans and discharge processes. Travel professionals will work independently with healthcare teams to support patient transitions.

Key Responsibilities:
  • Coordinate patient care plans and discharge processes to ensure seamless transitions
  • Conduct referral intakes for patients based on insurance and medical needs
  • Collaborate with healthcare teams to develop individualized care strategies
  • Maintain accurate documentation of patient interactions and care coordination activities
  • Execute follow-up procedures to monitor patient progress and adherence to discharge instructions
Qualifications:
  • Valid Montana RN license
  • BLS certification
  • Minimum of 2 years care coordination or discharge planning experience
  • Experience with Medicare, Medicaid, or other insurance types
  • Experience with referral intakes
  • Strong bedside manner
  • Ability to adapt to varying schedules (days, nights, or a combination)
  • Excellent communication skills
  • Ability to work independently
