Job Summary
Mass General Brigham's Compass Plus program is a multidisciplinary care service that supports patients with complex healthcare needs through care delivered in home and community settings. The program brings together providers, nurses, behavioral health specialists, and other care team members to deliver coordinated, patient-centered care focused on improving health outcomes, enhancing access to services, and supporting long-term health management.
We are seeking a full-time, 40-hour Nurse Practitioner to join the Compass Plus team, caring for a panel of approximately 120 patients throughout the Northern Massachusetts region, including but not limited to Lawrence, Lowell, Peabody, and Newburyport. The schedule for this role is Monday through Friday, 8:30 AM to 5:00 PM, with no nights, weekends, or on-call responsibilities.
This is a primarily remote position that requires regular local travel to patients' homes, primary care offices, and other community-based settings. The selected candidate will receive field-based onboarding and training, as well as all necessary equipment, including a laptop, cell phone, GPS safety device, and patient care supplies. In addition, attendance at a monthly in-person team meeting, as well as other occasional trainings and meetings at our Assembly Row office in Somerville, is required.
Summary
The Nurse Practitioner (NP) is a licensed provider. The NP is responsible for assessing and managing various patient populations, including those with medical and/or surgical conditions, as well as providing diagnostic and therapeutic interventions, developing appropriate plans of care, and conducting ongoing evaluations.

Essential Functions
-Provides direct care, counseling, and education to a designated patient population in ambulatory, inpatient, operative, and/or procedural settings.
-Performs complete histories and physical examinations.
-Orders, interprets, and evaluates appropriate laboratory and diagnostic tests.
-Develops appropriate plans of care and follow-up based on the outcomes of diagnostic, laboratory, and physical examination findings.
-Consistently provides high-quality and timely documentation, including admission and progress notes, procedure notes, operative notes, and discharge summaries.
-Performs bedside procedures as are appropriate to the patient population.
-Orders medications and writes prescriptions in accordance with organizational and regulatory policies and procedures.
