About the Role:

The Community Support Provider plays a vital role in enhancing the quality of life for individuals within the community by offering personalized assistance and support services. This position focuses on fostering independence, promoting social inclusion, and ensuring access to necessary resources for community members. The Community Support Provider acts as an advocate, helping individuals navigate social services, healthcare, and other community resources.

Minimum Qualifications:

  • High school diploma, GED, or the ability to pass a Comprehension Test and provide 3 non-family letters of reference.
  • Experience working in a community support, social services, or related role.
  • Valid driver’s license.
  • Ability to pass background checks and comply with state and federal regulations.
  • Strong communication and interpersonal skills.

Preferred Qualifications:

  • Able to speak and write both English and Spanish.
  • Experience working with people with Intellectual and Developmental Disabilities.
  • Ability to communicate effectively.
  • Ability to write clearly and document all contacts.
  • Ability to work effectively with individuals with special needs.
  • Ability to work under conditions requiring physical stamina and ability to work in a stressful environment.
  • Must possess good computer skills.

Responsibilities:

  • Provide direct support and assistance to individuals in their homes and community settings to promote independence and well-being.
  • Assist clients with daily living activities, including but not limited to personal care, transportation, and accessing community resources.
  • Monitor client progress and document services provided, ensuring compliance with organizational policies and regulatory requirements.
