Developmental Services Consulting Registered Nurse - Monday-Friday, Daytime

Community Care Network is looking for a Developmental Services Consulting Nurse to join our team!

Community Care Network is comprised of Rutland Mental Health Services and Rutland Community Programs. Our mission is to enhance the well-being of our communities, individuals and families through responsive, innovative and collaborative human services.

About the Role:

The Developmental Services Consulting Registered Nurse (RN) is a Monday-Friday, daytime position, and is responsible for the management and oversight of individual's health care needs. The DS Consulting RN advocates for health supports that reflect the individual's goals, preferences, and personal history in collaboration with individuals, families, guardians, case managers and the overall Team. The DS Consulting RN supports team planning, incident response, and transitions in care, in addition to promoting preventative health practices, managing chronic conditions, and facilitating access to primary and specialty care. The DS Consulting RN provides training and ensures all delegated tasks comply with the Vermont Board of Nursing standards, ensuring accountability and clinical integrity, as well as adherence to CCN policy and protocol.

Responsibilities:

  • Assess health status of individuals.
  • Establish Nursing diagnosis, as applicable, and goals related to health care needs.
  • Plan and implement strategy of medical and health care, prescribing Nurse interventions.
  • Delegate nursing interventions that may be performed by others.
  • Evaluate response to intervention and oversee step down care.
  • Collaborate with other health professionals and team members to ensure high quality training and oversight of chronic conditions.
  • Complete Emergency Facts Sheets, a minimum of annually, or as changes occur.
  • Update and document Special Care Procedures a minimum of annually, or as changes occur.
  • Monitor and verify accuracy of medication administration for contractors and staff.
  • Educate and train providers and staff regarding care of specific health conditions and other pertinent medical topics, as needed.
  • Monitor and coordinate Team compliance of the Health and Wellness Standards, Medicaid Guidelines, and other pertinent State and Federal regulation and rule.
  • Interpret and monitor medical record information impacting quality of life/care of individuals receiving Developmental Services.
  • Document clinical recommendations, communication and assessments timely and in compliance with CCN standards and requirements.
  • Ensure service delivery and billing practices consistently meet or exceed agency-defined productivity and fee-for-service requirements.
  • Provide nursing assessment of psychiatric status of individuals to providers for psychiatric medication checks.
  • Consult with teams before and after medical care appointments to monitor and assess quality care routines.

Qualifications:

  • Valid State Licensed Registered Nurse.
  • One year of nursing in the field of ID preferred.
  • Proficiency with computers, experience entering data into electronic medical records (EMR) program.
  • Ability to work with others in a positive, responsible manner.
  • Strong writing and organizational skills in order to receive and convey information in efficient and effective manner.
  • Ability to maintain confidentiality.
  • Flexibility, dependability, and ability to work independently.
  • Commitment to community inclusion and individual choice.
  • Able to flex schedule as needed.
  • Must have a valid driver's license, reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont.
  • Successful completion of a criminal history background check after hire
