Description
POSITION: Nurse
REPORTS TO: Executive Director
JOB SUMMARY: We are seeking a compassionate nurse professional to work in our dynamic, supportive and collaborative environment where both residents and employees thrive. The Community Nurse coordinates the health care services for assigned residents. Participates in individual service planning as well as renewal and maintenance of these plans. Recommends health care service goals and assists in their implementation. Oversees and coordinates work of House Managers and care givers to meet resident needs. Provides clinical services as needed to residents and trains staff to meet resident needs. Coordinates delegation of medication administration / assistance and health care tasks per state regulations, Nurse Practice Act.
The Community Nurse's duties include:

  • Represent Frontida with professionalism, always embracing Frontida's culture, which includes "treating others as you would like to be treated."
  • Works closely with the Executive Director to counsel the resident and designated agent(s) meeting personal and related health care needs. Notifies physician and/or designated agent(s) of resident status/change as appropriate.
  • Supervises and assists in provision of all Resident services, including the coordination of those provided by independently contracted ancillary health care providers as requested by Residents and families. Supervises and performs resident services (e.g., health care monitoring, nail care, medication assistance, etc.) based on individual resident service plans. Coordinates the healthcare service on a 24 hour/7-day week basis for assigned residents.
  • Conducts evaluations, individual service plans, develops service schedules and updates service information as indicates by Resident need. Addresses physical, social, spiritual, and psychological needs of residents.
  • Ensures that Residents are appropriately assessed on a timely basis and charged in accordance with that assessment.
  • Examines and gives first aid to residents and staff, within scope of practice.
  • Acts as an advocate for Residents and maintains communication with designated agents as requested and required.
  • Assures quality of Resident services.
  • Assures a high degree of customer satisfaction at home. Promptly investigates complaints and reports findings and appropriate recommendation to the Executive Director. Ensures follow-up communication occurs to complainant and that substantiated complaints are resolved to reasonable satisfaction of complainant.
  • Ensures compliance with all rules and regulations related to Resident care (i.e., HIPAA, State Regulations and OSHA, etc.).
  • Maintains a professional demeanor with all Residents and their loved ones.
  • Administers medications and treatments, based on individual resident service plans; observes results and documents same. Run reports to monitor electronic health record compliance.
  • Evaluates health emergencies and determines emergency medical measures taken regarding resident care.
  • Conducts and/or coordinates various health care "clinics" for residents.
  • Coordinates move-out of residents with Executive Director, as assigned.
  • Completes requested forms to ensure continuity of care.
  • Reads staff communications and maintains familiarity with other resident records as required.
  • Assures accurate transcription of physician orders into treatment/medical record.
  • Assists with move-in and orientation of new residents and designated resident(s).
  • Reviews resident service records, treatments, and medication books to assure quality of care.
  • Verifies physician orders, move in / move out information and initiates appropriate action/follow up.
  • Supervises ordering of medications from pharmacy.
  • Coordinates therapeutic diet orders with staff as required.
  • Assures proper records are maintained. Documents nurse's notes and change of condition.
  • Identifies resident and staff training needs.
  • Conducts training as assigned and mandated by regulatory requirements.
  • Orients and instructs care givers.
  • Adheres to and conveys philosophy of supporting dignity, privacy, independence, choice, individuality, and a home like environment for the residents.
  • Plans, analyzes, and evaluates the needs of residents and reports needs appropriately.
  • Coordinates, in cooperation with the Executive Director, ancillary health care services for residents.
  • Coordinates delegation of medication administration/assistance and health care tasks per state regulations and Nurse Practice Act.
  • Adheres to all established policies and procedures.
  • Responds to resident or staff concerns and responds appropriately including investigating, resolving, and/or referring them to the Executive Director.
  • Maintains statistics requested by management and submits reports as required.
  • Assists in maintaining good relations, participates in marketing, life enrichment and assisting in recruiting staff.
  • Ensures residents and staff receive Mantoux tests in the most appropriate, cost-effective manner.
  • Monitors quality of work performance of care givers and reports to Executive Director.
  • Assures infection control procedures are known/followed by staff.
  • Attended service plan conferences, maintained service and treatment records for each resident as required.
  • Works closely with staff and designated agent(s) to identify resident problems/concerns/issues; follows up as needed and communicates same appropriately.
  • Is on call according to pre-established schedule; refers for emergency care as appropriate.
  • Provides treatment within the scope of license, based on individual resident service plans. Coordinates ancillary services for complex health care needs with Executive Director and resident as assigned.
  • Adhere to and carry out all policies and procedures.
  • Establish and maintain constructive working relationships.
  • Observes all work, safety, and administrative rules.
  • All other duties as assigned.

Requirements
QUALIFICATIONS: Must complete probationary period successfully. Needs effective supervisory skills. Must be able to communicate and work with all levels of the company's Employees effectively. Have the ability to relate to the public, Residents, families, Employees, and other professionals appropriately. Ability to teach and train effectively. Willingness to adhere to assisted living principles in providing services. Ability/ willingness to perform all position responsibilities adequately and apply basic management principles of a Registered Nurse with a minimum of at least one year in home health or geriatric nursing in a similar position. Rehabilitation and restorative experience helpful. Graduate of an accredited school of nursing; current Registered Nursing License in the state in which you are employed Must have a history of actively demonstrating Frontida's culture and exhibiting extraordinary care toward Frontida's residents.
PHYSICAL REQUIREMENTS:
  • Must have the ability to regularly sit, occasionally required to kneel, crouch, and stand.
  • Must be able to travel to the office and work frequently on a computer.
  • Ability to lift up to 50 pounds, 30 pounds regularly.
  • Is free of non-treated communicable diseases.
  • Must be able to read, write, and communicate effectively.
