Job description

JOB SUMMARY


Reporting to the Director of CalAIM, the Community Supports Program Manager will be responsible for development, maintenance, and growth of the program, Community Supports. The Program Manager will help manage and oversee day-to-day operations of Community Supports, which includes housing transition navigation services, housing tenancy and sustaining, housing deposits and Asthma Remediation services. The Program Manager will provide programmatic oversight of Community Supports staff, including Lead Coordinators, Housing Care Managers, Housing Navigators, and Outreach Specialists, under the direction of the Director of CalAIM.

ESSENTIAL DUTIES AND RESPONSIBILITIES


Performs a combination, but not necessarily all, of the following duties:


  • Ensure Community Supports services are delivered in accordance with DHCS CalAIM requirements, health plan guidelines, and organizational policies.
  • Provide overall leadership and oversight of the Community Supports Program, including day-to-day operations, service delivery, quality measures, performance monitoring, and reporting requirements.
  • Support the development, expansion, and continuous improvement of Community Supports services to meet member needs and organizational goals.
  • Oversee program operations to ensure compliance with CalAIM, health plan, contractual, and organizational requirements.
  • Collaborate with the Director of CalAIM to support effective supervision and development of Community Supports staff.
  • Facilitate and participate in team, multidisciplinary, and cross-functional meetings to support service coordination and program success.
  • Collaborate with providers, staff, leadership, patients, and community partners to support integrated service delivery and achieve program objectives.
  • Serve as a point of contact for internal and external stakeholders, including partner agencies, referral organizations, and health plans.
  • Ensure timely and accurate documentation of assessments, services, and member eligibility, including compliance with documentation standards and payer requirements.
  • Review, audit, and monitor staff documentation, charts, and service records to ensure quality, accuracy, and regulatory compliance.
  • Ensure required program and patient data are accurately entered and maintained within eClinicalWorks and other applicable systems.
  • Maintain current knowledge of community resources and referral options and support strong partnerships with community-based organizations.
  • Collaborate with the Finance Manager to monitor program budgets, process requisitions, and oversee other assigned financial and administrative functions.
  • Prepare, update, and submit required program reports, including staff capacity reports and health plan reporting requirements.
  • Coordinate, develop, and maintain Memoranda of Understanding (MOUs) and other partnership agreements with referral and community-based organizations.
  • Assist in the preparation of grant, contract, and program reports by coordinating data collection and reporting activities to demonstrate achievement of goals and objectives.
  • Develop and support program presentations, trainings, technical assistance resources, educational materials, and promotional tools for providers, staff, and community partners.
  • Collaborate with the Director of CalAIM, Development Team, Chief Medical Officer, and other organizational leaders to promote the integration and growth of Community Supports services.
  • Work with clinical and operational teams to integrate Community Supports and other designated programs into organizational workflows and care delivery models.
  • Identify, evaluate, and support strategic opportunities to enhance program effectiveness, member outcomes, and organizational impact.

Supervisory Responsibilities

  • Assist the Director of CalAIM in overseeing Community Supports staff and ensuring effective program operations and service delivery.
  • Participate in the recruitment, hiring, onboarding, training, coaching, performance management, and professional development of assigned staff.
  • Provide ongoing guidance, mentorship, and accountability to team members to ensure achievement of program goals, productivity standards, and quality expectations.
  • Conduct or participate in staff meetings, performance evaluations, corrective action processes, and other supervisory activities as delegated by the Director of CalAIM.
  • Support staff scheduling, workload management, and resource allocation to ensure continuity of services.
  • Review and approve staff CONCUR expense reports.
  • Oversee invoicing, billing, and other assigned fiscal and administrative responsibilities to support program operations and compliance requirements.

Program Evaluation and Quality Improvement

  • Collaborate with the Quality Improvement (QI) and Compliance Committee to measure, evaluate, and improve program effectiveness.
  • Develop, implement, and monitor performance measures, outcome tracking tools, and quality improvement initiatives.
  • Track, analyze, and report program outcomes and performance indicators as required by contracts, grants, health plans, and organizational leadership.
  • Conduct and analyze patient satisfaction surveys and other feedback mechanisms to support continuous quality improvement.
  • Coordinate and support internal and external program evaluations, audits, and monitoring activities.
  • Assist in the recruitment, coordination, and supervision of interns, partners, consultants, or other resources supporting evaluation activities.
  • Support the development of data-driven recommendations to improve program effectiveness, efficiency, and member outcomes.

Other Duties

  • Attend meetings, training courses, conferences, and other work-related events as required.
  • As an exempt position, occasional evening and/or weekend work may be required to fulfill program responsibilities.
  • Perform other duties as assigned and as business needs dictate.


QUALIFICATIONS


Education and Experience:


  • Bachelor’s degree in nursing, medical, social work, sociology, psychology, public health, or health related field preferred
  • Minimum of two (2) years of progressively responsible experience in program management, supervision, quality improvement, or related healthcare/social services leadership roles.
  • One (1) year related experience working in medical and/or social services
  • Research experience in health services or social science research.
  • Working knowledge of health information systems and data needed to monitor/assess utilization, performance management, and health outcomes.
  • Experience preparing and delivering presentations.
  • Knowledge of development of evaluation tools, and evaluation of health programs, data analysis, and report writing.
  • Strong health care knowledge base required to strategically develop health program evaluations and manage the contract(s);
  • Knowledge of community health evaluation tools, approaches, frameworks, and methods.
  • Grant/contract budgeting knowledge.
  • Bilingual English/Spanish preferred (read, write, speak)

  • Must have reliable transportation to travel between Los Angeles County, Riverside County and San Bernardino County
