Community Outreach Specialist (Make a Real Impact)

Full-Time | Monday–Friday | No Weekends

Are you passionate about helping older adults live safely and independently? Do you love building relationships and being out in the community?

We’re looking for a driven, people-focused Outreach Professional to grow our presence and connect seniors and families to life-changing care.


What You’ll Do:
  • Build relationships with healthcare providers, senior communities, and local organizations

  • Educate families on care options and guide them through the enrollment process

  • Represent our organization at community events and networking opportunities

  • Generate and manage referral leads to support program growth

  • Partner with internal teams to ensure a smooth and supportive onboarding experience
