Position Purpose

The Chief Executive Officer (CEO) of Aviva Health provides visionary, mission-driven leadership to ensure the delivery of high-quality, integrated healthcare services to the rural communities of Douglas County. As the leader of a Federally Qualified Health Center (FQHC), the CEO is responsible for advancing access to comprehensive, patient-centered care-including medical, dental, and behavioral health services-while addressing the unique challenges of a rural and underserved population.


The CEO partners closely with the Board of Directors to guide strategic direction, ensure financial sustainability, and maintain compliance with all Health Resources and Services Administration (HRSA) Health Center Program requirements. This role requires a strong commitment to health equity, community engagement, and innovative solutions to the workforce and access barriers common in rural healthcare settings.


Key Responsibilities

Strategic Leadership & Board Governance Support

  • Partner with the Board of Directors to define, execute, and regularly refresh Aviva Health's strategic plan, ensuring alignment with the mission, community needs, and FQHC requirements.
  • Attend meetings of the Board and its committees and workgroups, supporting them with requested or necessary information and data.
  • Prepare Board meeting materials and provide the Board with summary reports-and more detailed reports on request-covering patient census, financial activity, HR and personnel activity, medical staff credentialing status, medical/dental/behavioral health patient activity, compliance activity, and quality improvement/quality assurance, both at meetings and as needed between meetings.
  • Support effective governance practices, including policy development, Board engagement, and assistance with recruitment, orientation, and retention of a Board that is representative of the patient population.
  • Lead long-range and strategic planning to expand access and services across a geographically dispersed rural region.
  • Carry out additional duties as delegated by the Board of Directors.


Regulatory Compliance, Risk Management, & FQHC Program Requirements

  • Ensure full, organization-wide compliance with HRSA Health Center Program requirements and the conditions of Aviva Health's Section 330 award/designation, including successful Operational Site Visits (OSVs).
  • Maintain compliance with the Sliding Fee Discount Program, Uniform Data System (UDS) reporting, Federal Tort Claims Act (FTCA) deeming requirements, and the 340B Drug Pricing Program, as applicable.
  • Ensure timely submission of HRSA applications and reports, and obtain HRSA prior approval for changes in scope of project, key management staff, or other matters requiring it.
  • Maintain compliance with all applicable federal, state, and local regulations (e.g., HIPAA, OSHA, CMS, Oregon Health Authority).
  • Establish, interpret, and maintain policies and procedures that sustain compliance; review them at least annually and explain them to staff, patients, and the Board as needed.
  • Champion and oversee enterprise risk management, corporate compliance, and quality assurance programs.
  • Maintain a current knowledge base of Community Health Center industry trends and regulatory change.


Rural Health & Community Impact

  • Champion equitable access to care for underserved and vulnerable populations, including uninsured, underinsured, and geographically isolated individuals.
  • Develop innovative strategies to address rural healthcare barriers such as transportation, provider shortages, and social determinants of health.
  • Build and sustain partnerships with local hospitals, public health agencies, schools, and community-based organizations.
  • Serve as a visible and engaged leader within Douglas County and the broader regional healthcare community.


Operational & Clinical Oversight

  • Provide executive oversight of integrated care delivery, including medical, dental, and behavioral health services.
  • Partner with the Chief Medical Officer to ensure high standards of clinical quality, patient safety, and continuity of care, and to support the maintenance of health care standards across the organization.
  • Support care models that enhance access, such as team-based care, telehealth, and outreach services.
  • Ensure operational efficiency across multiple clinic sites and service lines, and regularly assess the physical environment and plant operations of health center sites.
  • Maintain general charge of the corporation's assets and supervise and control all employees of the organization.
  • Assure compliance with all Board policies, including the credentialing and privileging of licensed employees and contractors.


Financial Stewardship

  • Partner with the CFO and Finance Committee to prepare Aviva Health's annual operating and capital budget; present a draft for Board consideration and the final budget for approval no later than June 30 (fiscal year July 1-June 30).
  • Ensure financial sustainability through diversified funding streams, including Section 330 funding, Medicaid/Medicare reimbursement, grants, and local support.
  • Monitor financial performance and key indicators; implement strategies to address the margin pressures common in rural healthcare; and oversee revenue cycle optimization and cost management.
  • Review and sign operational and routine contracts on behalf of the corporation; co-sign significant contracts with the Board President as required by the by-laws; and sign or countersign drafts, notes, and other undertakings for the payment of money as authorized by the Board. Assure all agreements are legally and financially sound and do not impinge on the authority of the Board.
  • Authorize the purchase of capital equipment and supplies as approved by the Board in the annual budget.
  • Acquire and maintain adequate insurance protecting the corporation's assets, Directors, Officers, employees, and volunteers, and maintain the fidelity bond required by the by-laws.


Workforce & Human Resources

  • Hire all other employees and agents of the corporation and determine their titles, terms of employment, duties, and compensation; develop and maintain a sound organizational structure and staffing pattern; and direct, evaluate, and, as necessary, terminate staff.
  • Lead innovative recruitment and retention strategies to address rural workforce shortages, particularly for licensed clinical providers.
  • Foster a culture of engagement, accountability, and inclusion across a multidisciplinary workforce, and oversee leadership development, succession planning, and performance management.
  • Participate in and conduct in-service training and ensure compliance with employment laws and HR best practices.


Community Engagement, Advocacy, & Fund Development

  • Serve as Aviva Health's primary spokesperson and advocate at the local, state, and regional levels, and represent the organization for media, news, publicity, and promotional events.
  • Strengthen community trust through active engagement, transparency, and responsiveness.
  • Partner with organizational leaders to lead fundraising efforts, including grant development, donor cultivation, and community campaigns.
  • Advocate for policies that support rural healthcare access and FQHC sustainability.


Quality Improvement & Patient Experience

  • Champion a culture of continuous quality improvement and patient-centered care.
  • Oversee performance on Uniform Data System (UDS) metrics and other clinical quality measures.
  • Support the Chief Medical Officer-led QA/QI Committee and ensure provider credentialing and privileging recommendations are advanced for Board approval.
  • Ensure protection of patient rights, dignity, and confidentiality.
  • Promote initiatives that improve health outcomes and the patient experience across diverse populations.


Qualifications

Required

  • Bachelor's degree in Healthcare Administration, Public Health, Business Administration, or a related field.
  • Minimum of 5-7 years of senior leadership experience, preferably in healthcare in a community health center or rural health setting.
  • Experience working effectively with a governing Board of Directors.
  • Strong financial acumen, including budgeting, forecasting, and managing complex funding streams.

Preferred

  • Master's degree (MHA, MPH, MBA, or related field).
  • Demonstrated knowledge of HRSA Health Center Program requirements and FQHC operations.
  • Experience leading in a rural or frontier healthcare environment.
  • Demonstrated success in provider recruitment and retention strategies.
  • Experience with integrated care models and value-based care initiatives.
