Your future role at a glance

Location:  Remote with up to 10% travel

Department:  Community Impact

Schedule:  Day shift | Full-time

Salary:  $107,821.50 - $145,875.45 (per year)

Eligible for annual bonus incentive

How you'll make an impact in this role

The Director of Community Impact drives organizational success by establishing robust relationships with vendors, Federally Qualified Health Centers (FQHCs), and key community health stakeholders. This role oversees the full lifecycle of strategic FQHC initiatives-from initial planning and design through implementation and evaluation-to improve community health outcomes.

Key Responsibilities:

  • Strategic Leadership: Direct the planning, execution, and evaluation of strategic FQHC initiatives to align with broad organizational community health goals.
  • Partnership Development: Cultivate and maintain high-impact relationships with FQHCs, external stakeholders, and community partners to promote collaborative care models.
  • Operational Support: Provide dyadic support to the Nurse Director, facilitate monthly market collaborative meetings, and implement robust tracking systems to monitor relationship health and program success.
  • Cross-Functional Collaboration: Foster strong working relationships across internal departments and with community stakeholders to deliver high-value, differentiated services.
What minimum qualifications you'll need

Education:

  • High School diploma equivalency with 5 years of applicable cumulative job specific experiencerequired, with 2 of those years being in leadership/management OR Associate's degree/Bachelor'sdegree with 3 years of applicable cumulative job specific experience required, with 2 of those yearsbeing in leadership/management.
What additional requirements you'll need
  • Education: Master of Business Administration (MBA) or Master of Health Administration (MHA) preferred.
  • Experience: 5+ years of relevant experience within FQHC, non-profit healtcare or Public Health environments.
  • Knowledge: Firm understanding of population health management, value-based care, and contracting models.
  • Skills: Proven expertise in market relationship tracking and collaborative program management.
