Job Description
Works as a liaison to grant support to the vulnerable members of the community, addressing material and health challenges. Maintains a caseload of members and provides referrals to various support services by conducting assessments and in home visits. Serves as a role model and will identify and initiate referrals to social service programs, serve as main point of contact for the member and support the implementation of various care activities.
Job Responsibility
1.Serves as a mentor for Community Health Workers
2.Provides oversight in care of members to improve health outcomes and promote well-being
3.Acts a resource for CHW and members
4.Provides support and basic health and education about material and general health care and community resources.
5.Conducts one-on-one health education, community outreach, regular in-home visits and service coordination.
6.Provides various supports, services, entitlements and benefits for members.
7.Supports members in developing health management care plans.
8.Empowers members by promoting effective approaches, problem solving and lifestyle changes.
9.Works to establish positive and support relationships and effective works with a diverse group of members.
10.Delivers coaching and mentorship on chronic health conditions and self-care.
11.Enters client data into software on a daily basis.
12.Advocates for members and their families and collaborates with the care team as needed.
13.Perform all related duties, as required. All responsibilities noted here are considered essential functions of the job under the American with Disabilities Act. Duties not mentioned here, but considered related are not essential functions.
Job Qualification
• High School Diploma or equivalent required.
• 1-3 years of relevant experience, required.
Community Health Worker certificate, highly preferred.
