Seeking Two Part-Time Community Health Workers (CHWs) – Family Home Visiting Research Project

Role Overview: We are seeking two part-time extremely responsible Community Health Workers (CHWs) who are passionate about coaching families with preschool aged children, supporting their adoption of healthy family routines and behaviors. These CHW roles involve delivering home visiting services to families over a six-month period, following a curriculum modified from the NET-Works study (https://pubmed.ncbi.nlm.nih.gov/37171137/). This CDC-funded research project is a partnership with the University of Minnesota and will examine the model’s implementation with families referred from three Twin Cities Federally Qualified Health Centers (FQHCs). Outcomes and impact will be measured through standardized data collection. CHWs will conduct visits in patients’ homes following best practices.


Key Responsibilities:

  • Deliver a series of six monthly home visits (2-3 home visits per week) to families with preschool aged children referred from Neighborhood HealthSource and Southside Community Health Services clinics.
  • Work with families to set goals and implement routines (meal and snack time, playtime and bedtime).
  • Utilize Motivational Interviewing, positive parenting skills and other coaching methods to help families adopt individualized and culturally relevant routines.
  • Engage families in interactive activities supporting child development and active play.
  • Facilitate 2-3 check-ins with families weekly between home visits via telephone, Zoom, email, or text messages.
  • Document home visits in CHW Solutions’ Electronic Health Record.
  • Gather research related information and input it into a University of Minnesota REDCap electronic database.
  • Deliver health promotion topics and connect families to community resources in culturally and linguistically relevant ways.
  • Complete all required research training, including CITI IRB, REDCap electronic database and NET-Works intervention curriculum.
  • Attend biweekly team meetings (approximately 1 hour) with University of Minnesota research staff to debrief on home visits and the overall implementation process, and to update study protocol as needed.


Additional Responsibilities:

  • Coordinate with University of Minnesota research staff and CHW Solutions’ leadership to assure home visits follow a standardized model based on the NET-Works study.
  • Assist company Co-Founders and CHW Services Director to refine existing health education practices to develop effective models of patient care.
  • Complete the MDH CHWs 101: Introduction to Research training to engage in other research activities.


Required Qualifications:     

  • Excellent communication skills with patients, co-workers, and partners.
  • Experience delivering health education services.
  • Driver’s License and reliable vehicle.
  • Willingness to drive to home visits within 45 minutes (one way) of the Twin Cities Metro Area.
  • Fluency in Spanish and/or Somali and English.
  • Availability to work one or two evenings (until 7:00 PM) per week as needed.


Preferred Qualifications:

  • CHW certificate from one of the Minnesota Community Health Worker training programs. (CHW Solutions will consider supporting applicants to obtain their CHW certificate while employed at CHW Solutions.)
  • At least one year of direct patient contact as a CHW or similar role.
  • Experience providing home visiting.
  • Experience documenting in electronic health records and following HIPAA and other security procedures.
