The Fund for Public Health in New York City (FPHNYC) is a 501(c)3 non-profit organization that is dedicated to the advancement of the health and well-being of all New Yorkers.ย  To this end, in partnership with the New York City Department of Health and Mental Hygiene (DOHMH), FPHNYC incubates innovative public health initiatives implemented by DOHMH to advance community health throughout the city. Itย facilitates partnerships, often new and unconventional, between government and the private sector to develop, test, and launch new initiatives. These collaborations speed the execution of demonstration projects, affect expansion of successful pilot programs, and support rapid implementation to meet the public health needs of individuals, families, and communities across New York City.

About The Role: Equitable Health System (BEHS) is seeking aย Communityย Healthย Worker (CHW), who is a detail-oriented individual with knowledge of the New York Cityย socialย service landscape, data-management, computer, and critical thinking skills to supportย implementation of the Falls Prevention Projectby engaging older adults(50+) and adults with disabilitiesat elevated risk of falls,facilitatingaccess to home repair and supportive services, and promoting safe aging in place.ย The CHW will serve as a trusted liaison between participants, healthcare providers, and community-based partners to ensure coordinated service delivery, reinforce falls prevention education, and reduce preventable hospitalutilizationrelated to falls. Through outreach, screening, care coordination,including home visitsand data collection, the CHW will advance program goals focused on improving home safety, enhancing participant independence, and supporting evaluation and reporting requirements.ย About the Program:

Be a change agent and join the Bureau of Equitable Health Systems (BEHS), a bureau in the NYC Department of Health and Mental Hygiene.ย ย The Bureau of Equitable Health Systems unifies several units to strengthen the Departmentโ€™s ability to strategically partner with the NYC healthcare system (including but not limited to integrating behavioral health and community linkages into primary care practice). The bureau will engage primary care providers, hospitals, and other healthcare systems to implement evidence-based strategies; leverage information to support planning and technical assistance for providers and payers; advance policy to close the racial equity gap for priority health outcomes; and surface opportunities where health care can influence and connect individuals to social support and address the whole person, beyond physical ailments.ย ย 

Responsibilities:

Outreach, Engagement & Referralย 

  • Conduct targeted outreach in high-need communities and healthcare settings toย identifyย and engage eligible participants.ย 
  • Build relationships to support enrollment; clearly communicate program services, eligibility, and next steps using culturally and linguisticallyย appropriate approaches.ย 
  • Provide education on fall prevention, safe home practices, and aging in place; share relevant resources.ย 
  • Screen for fall risk and home safety needs and enroll eligible participants.ย 
  • Develop andย maintainย referral partnerships; collaborate with providers and community organizations to increase awareness and connect participants toย additionalย services.ย 
  • Other duties as assigned to support the overall goals and mission of theย department/ organization.

Care Coordination & Home Visitsย 

  • Coordinate and schedule home visits with internal teams and contractors.ย 
  • Support and, whenย appropriate,ย accompanyย home assessments/modifications.ย 
  • Educate participants on recommended home modifications.ย 
  • Communicate referral status and updates with partners asย appropriate.ย 

Program Operations, Data & Team Collaborationย 

  • Collect and manage participant data, program metrics, and service documentation.ย 
  • Administer pre/post assessments and support program evaluation and reporting.ย 
  • Participate in training, supervision, and case conferences.ย 
  • Collaborate with internal teams and external partners to ensure effective program delivery.ย 
  • Other duties as assigned to support the overall goals and mission of the department/ organization.
Requirements:

MINIMUM QUALIFICATION REQUIREMENTS:

  • High school diploma or GED required, bachelorโ€™s degree preferred.ย 
  • At least 1 year of experience in community-based clinical or social service settings.ย 
  • Fluency in English; bilingual Spanish preferred.ย 
  • Familiarity with NYC community and faith-based organizations.ย 
  • Willingness to work evenings/weekends and travel throughout NYC.ย 
  • Proficiencyย with basic computer systems.ย

  • Strong interpersonal and communication skills; ability to engage diverse populations with cultural competence.ย 
  • Ability to work both independently and collaboratively in team-based, cross-disciplinary environments.ย 
  • Strong organizational, time-management, and problem-solving skills; able to manage competing priorities and deadlines.ย 
  • Demonstrated advocacy and community engagement skills.ย 
  • Interest or experience in programs addressing chronic illness and social determinants of health.
