POSITION SUMMARY:
The Community Health Coach facilitates and leads one or more YMCA Community Health Programs including, but not limited to YMCA's Diabetes Prevention, Blood Pressure Self-Monitoring and Empower Post Stroke Program. May also provide biometric screenings, health coaching and assist in participant recruitment and administrative duties, as assigned. This position supports the work of the Y, a leading nonprofit committed to strengthening community through youth development, healthy living and social responsibility.

ESSENTIAL FUNCTIONS:
1. Ability to display empathy towards health seekers with chronic disease
2. Provide motivation, support and guidance to program participants who are engaging in healthy behavior changes within the YMCA's Community Health Programs
3. Ability to follow strict program delivery guidelines with consistency
4. Use of strong facilitation skills that include Listen First/Motivational Interviewing concepts
5. Ensure participant commitment and retention while achieving quality performance outcomes
6. Prepare before each session and follow-up with participants in-between sessions
7. Keep accurate attendance records and program data, reporting data within required timeframe and databases
8. Maintain participant confidentiality by managing program records and personal health information according to YMCA's HIPPA organizational policies
9. Participate in all Community Health team discussions (meetings, emails, and conference calls) as directed by supervisor
10. Create a safe, judgment-free, and encouraging environment that is friendly and non-competitive for participants
11. Build effective, authentic relationships with program participants, connecting them with each other and the YMCA
12. Perform all other duties as assigned

QUALIFICATIONS:
1. College degree in Public Health, Health Education, Exercise Science or related field preferred
2. At least 18 years of age
3. Must comply with organizational policies
4. Experience in the implementation and facilitation of health and wellness or public health programs required
5. Must possess health coaching skills and Listen First/Motivational Interviewing skills
6. Must have access to reliable transportation to be able to travel to multiple off-site class locations
7. Proficient in computer use and Microsoft Office suite
