Reports To

Vice President, Business Development

Position Summary

Under the direction of the Vice President of Business Development or designee, the Business Development Manager is responsible for driving strategic business growth across The New Jewish Home's continuum of care, including Skilled Nursing Facilities (SNF), Certified Home Health Agency (CHHA), Solutions at Home, Day Centers, rehabilitation services, and future service lines.

The position combines strategic business development with direct patient referral management in collaboration with the admissions team.  Cultivating referral relationships, coordinating transitions of care, and serving as the primary liaison between hospitals, physicians, community organizations, patients, families, and internal clinical teams is a key component of this position.

This role is responsible for expanding market presence, identifying new business opportunities, strengthening referral partnerships, improving care transitions, and ensuring exceptional customer service throughout the admissions process.

Essential Duties & Responsibilities

ยท       Develop and execute strategies to drive census growth across all New Jewish Home service lines.

ยท       Identify, cultivate, and secure new referral sources including hospitals, physician groups, skilled nursing facilities, community organizations, and healthcare providers.

ยท       Build long-term relationships with referral partners to increase admissions and market share.

ยท       Collaborate with internal leadership to identify cross-selling opportunities among business units.

ยท       Promote all New Jewish Home programs through networking, presentations, committee participation, conferences, and community outreach.

ยท       Assist with development and implementation of new business initiatives and service expansion projects.

ยท       Coordinate communication and transition-of-care initiatives between departments to improve patient outcomes.

Patient Care Liaison & Admissions

ยท       The Patient Care Liaison reports directly to the Business Development Manager.

ยท       Conduct hospital and facility visits throughout the metropolitan area to evaluate potential admissions in conjunction with admissions team

ยท       Serve as the primary liaison between referral sources, patients, caregivers, hospital staff, and New Jewish Home clinical teams.

ยท       Facilitate the admissions process from referral through acceptance.

ยท       Complete admissions applications with patients and families via telephone or in person.

ยท       Educate applicants and families regarding available programs and services.

ยท       Monitor hospitalized residents and coordinate appropriate return-to-facility planning when applicable.

Relationship Management

ยท       Maintain strong working relationships with hospitals, physicians, discharge planners, social workers, case managers, and community agencies.
