This role is set within an assisted living facility in Sparks, Nevada, where you will work onsite alongside a multidisciplinary team of healthcare professionals. The facility offers a supportive atmosphere that encourages teamwork and comprehensive patient-centered care. Your schedule will typically run from 9:00 a.m. to 6:00 p.m., with the possibility of evening clinics until 8:00 p.m., ensuring you can accommodate various patient needs.

• Conduct thorough health assessments and screenings for residents, identifying both acute and chronic medical conditions to formulate effective treatment plans.
• Diagnose and manage a wide range of health issues while providing preventive care and health promotion strategies tailored to individual patient needs.
• Collaborate with a network of community organizations and healthcare stakeholders to implement health education initiatives aimed at improving knowledge and access to essential services.
• Deliver culturally sensitive care while advocating for the rights of residents and ensuring equitable access to healthcare resources.
• Coordinate referrals and follow-up care to promote continuity and comprehensive management of patient health.
• Maintain up-to-date electronic medical records and documentation, ensuring compliance with regulatory standards and organizational protocols.
• Engage in community outreach events and health fairs during weekends, focusing on health screenings and education to uplift the local community.
• Stay informed about current trends in community health, evidence-based practices, and relevant healthcare policies to support best practices in patient care.

This role primarily serves residents of an assisted living facility, encompassing a diverse population with varied healthcare needs. You will engage with elderly individuals, helping them manage chronic illnesses, promoting wellness and preventive health, and addressing the social determinants of health that impact their overall well-being.

• Current certification as a Nurse Practitioner with an active state nursing license in Nevada.
• A Master’s degree in nursing from an accredited program, ensuring a strong foundational knowledge of nursing principles.
• A minimum of 3-5 years of clinical experience, particularly in community health or public health settings, to demonstrate the ability to work effectively with diverse patient populations.
• Exceptional interpersonal and communication skills that allow you to connect with patients and healthcare teams alike.
• A solid commitment to social justice and health equity, actively advocating for the needs of vulnerable communities.

• Experience in geriatric care or working within assisted living environments to understand the unique challenges faced by this population.
• Prior involvement in community health education programs or initiatives, showcasing your ability to educate and empower patients and families.

• Competitive salary range of USD 120,000–130,000 annually, commensurate with your experience and qualifications.
