Job description

LVN Sub Acute Neuro Care

Community Hospital San Bernardino, San Bernardino, California

As our Licensed Vocational Nurse (LVN), you will provide vital direct patient care, significantly contributing to patient well-being and recovery as an integral part of our compassionate healthcare team. Every day, you will assess patient conditions, administer medications, perform prescribed treatments, and meticulously document care. You will collaborate with RNs and physicians to implement individualized care plans, ensuring a seamless and high-quality patient experience.

To be successful in this role, you will demonstrate exceptional clinical skills, a strong commitment to patient safety, and excellent communication. Your compassionate demeanor, attention to detail, and ability to prioritize in a fast-paced environment are essential.

  • Assures that the rights of the patient/family are respected and maintained
  • Provides for physical, psychosocial and spiritual needs of the patient/family
  • Provides for the education of the patient/family
  • Assists Registered Nurse in the Development, implementation and evaluation of the nursing plan of care for the patient/family
  • Incorporates the plan for nursing care into the multidisciplinary care and management of the patient/family
  • Demonstrates the ability to safely and competently administer medications, intravenous therapy and basic fluid management

Required

  • One (1) year experience in Long Term Care and/or Subacute required.
  • Valid California IV Certification through the Board of Vocational Nursing and Psychiatric Technicians (BVNPT) or to be obtained within 90 days of employment and will be contingent on continued employment
  • Basic Life Support (BLS) Certification
