Job description

Registered Nurse (RN)

Participate in the development and implementation of the service plan and initial assessments for clients receiving nursing services to ensure proper placement of staff

Regularly assesses the needs of clients receiving Nursing services

Assess and chart observation of the patient's condition at each visit

Dress or redress wounds and assess the healing process

Provide education to patients and families on proper home health care procedures and strategies

Coordinate with Physicians, Physical Therapist, Occupational Therapists, and other individuals in the patient's care plan

Provide hands on care, (i.e., wound care, ventilator care etc.)

Recommend tools or devices that may improve quality of life for the patients

Regular evaluations and re-evaluations (as warranted by changes in condition but at least every 60 days) of the patient's nursing needs

Taking verbal orders

Make referrals to other disciplines, as indicated by patient's assessment

Maintain a current and accurate patient medication profile

Medication Administration Training

Monitoring Medication

Complete service plans

Supervise CNAs and PCAs and Nurses

Train new staff

Supervise management of records

Investigate incidents/grievances

Investigate accidents

Conduct orientation training

Communicate timely and effectively with the agency on changes in visit assignment, dates of scheduled visits, and schedule changes to scheduler and patient care to ensure patient needs are met

Recurring training requirements and training staff based on assessments and requirements

Licensed Practical Nurse (LPN)

PRN/Per Diem

Deliver skilled nursing care under RN supervision

Provide medication administration, wound care, and patient education in clients' homes across the Lowcountry and Midlands

Requirements:

Minimum 2 years experience

Basic Health and Tuberculosis screening

Participate in Quality Improvement Program

Able to lift 50 Lbs.

Current RN licensure for State of practice

Current CPR

Ability to think and work independently in a fast-paced and sometimes stressful environment

Valid Driver's license, vehicle insurance

Meeting current training requirements

Work around your own schedule

PRN

Weekly payroll
