Job description

) Buena Vida Rehab and Nursing Center is actively seeking a full-time MDS Coordinator. We are seeking a detail-oriented and compassionate Registered Nurse (RN) to join our team as an MDS Assessor. The ideal candidate will have knowledge of the Minimum Data Set (MDS) process and be responsible for coordinating and completing accurate and timely MDS assessments in accordance with federal and New York State regulations. Previous MDS experience is preferred but not required.
Responsibilities Include
  • Complete MDS assessments in compliance with CMS and New York State regulations.
  • Ensure accurate and timely completion of MDS, Care Area Assessments (CAAs), and care plans.
  • Review medical records to ensure documentation supports MDS coding.
  • Support MDS related reimbursement processes.
  • Complete skilled Medicare certificate and issue Medicare notices (NOMNCs) in accordance with applicable guidelines.
  • Assist with quality improvement initiatives and survey preparedness.
  • Communicate effectively with physicians, nursing staff, therapists, residents, and families as needed.
    Qualifications
  • Current New York State Registered Nurse (RN) license in good standing.
  • Long-term care or skilled nursing experience required.
  • Some MDS experience preferred. (Knowledge of RAI/MDS 3.0 guidelines is a plus)
  • Strong assessment, documentation, and organizational skills.
  • Excellent communication and teamwork abilities.
  • Proficiency with electronic medical records (EMR) systems is required. (Point Click Care)
