Registered Nurse (RN) Assessment Nurse Job Description

Position Summary:
The Registered Nurse (RN) Assessment Nurse at Angels of Hope Homecare is responsible for conducting comprehensive in-home patient assessments to determine eligibility, level of care, and service needs. This role ensures that all clients receive safe, high-quality, and compliant care while supporting the agencys mission of compassionate, patient-centered service.
Key Responsibilities:

  • Conduct initial and ongoing in-home assessments for clients to evaluate physical, mental, and environmental needs.
  • Develop, implement, and update individualized care plans in accordance with physician orders and state regulations.
  • Accurately complete and submit all required documentation, including STAT assessments, within designated timeframes.
  • Ensure compliance with Indiana Medicaid Waiver, state regulations, and Angels of Hope Homecare policies and procedures.
  • Collaborate with caregivers, physicians, case managers, and office staff to coordinate effective care delivery.
  • Monitor client conditions and make recommendations for changes in care or services as needed.
  • Provide education and guidance to clients, families, and caregivers regarding care plans and health management.
  • Participate in quality assurance activities and support agency compliance standards.
  • Report any changes in client condition, incidents, or concerns promptly to the Director of Nursing.
Qualifications:

  • Current, active Registered Nurse (RN) license in the State of Indiana.
  • Minimum of 12 years of nursing experience (home health or assessment experience preferred).
  • Knowledge of Indiana Medicaid Waiver and homecare regulations is preferred.
  • Valid drivers license, reliable transportation, and auto insurance.
