Job description

Pharmacist Position at River Bend Hospital

Our mission at Meridian Health Services is to close the gap on access to Whole-Person Healthcare by expanding locations and services so that all people in underserved communities can get quality treatment. You are the dynamic, professional pharmacist we are looking for at River Bend Hospital, West Lafayette, IN. In a PRN capacity, you will use your experience and expertise in processes involved in dispensing and preparing medicines; ensuring compliance with FDA mandates and self-imposed manufacturer restrictions on particular drugs, as applicable; enforcing all federal, state and local laws and regulations pertaining to drug dispensing and patient privacy, as well as company policies and procedures. The hours for this position is 8a-5p in a PRN capacity.

Pharmacist key responsibilities include:

  • Prepares and dispenses medication for inpatients
  • Prepares and dispenses sample medications / Medication Assistance medications for outpatients as ordered by WVA prescribers
  • Prepares all extemporaneous prescriptions called for in the pharmacy
  • Will fill medication planners as requested for inpatients going home or outpatients as arranged with case managers
  • Responsible for safely storing medications left in the pharmacy for use in an individual's medication planners
  • Provides 24 hour emergency on-call services for inpatient pharmaceutical needs per policy and procedure
  • Completes written/computer records of each prescription filled for pharmacy including controlled medication file
  • Responsible for logging in medications brought to the hospital by patients upon admission (done by the nurse and witness after hours)
  • Responsible for seeing that all sample medications received in pharmacy are logged in, stored safely / properly and that all outdated sample medications are disposed of properly
  • Instructs individual patients on medication use as appropriate
  • Furnishes information about medications as requested for staff and patients
  • Provide in-services to staff and clients as requested regarding medication and associated information.
  • Responsible for overseeing ordering of pharmacy supplies and medications
  • Plan implement, maintain and supervise procedures for packaging and labeling pharmaceuticals according to policy and legal requirements, to ensure quality, security, and proper disposal
  • Responsible for reviewing all medication orders in compliance with the Joint Commission and state and federal laws
  • Provides clinically oriented medication distribution system
  • Provides clinical services offered by the department
  • Provide information and advice regarding drug interactions, side effects, dosage and proper medication storage
  • Collaborates with other healthcare professionals to plan, monitor, review and evaluate the quality and effectiveness of drugs and drug regimens
  • Establishes and coordinates pharmacy policies and procedures affecting other departments of the hospital
  • Primarily responsible for the establishment of policies and procedures with the department
  • Verifies receipt of medications & supplies ordered, approves bill for payment
  • Responsible for updating pharmacy computer software as updates become available OR if updates require IT assistance - responsible for contacting IT to do updates in a timely manner
  • Responsible for overseeing inventory of crash cart for River Bend Hospital (RBH), monitoring expiration dates and replacing used items promptly
  • Manages RBH / WVA formulary per policy & procedures
  • Maintains an inventory of pharmaceutical medications for use at River Bend Hospital based on WVA / RBH formulary
  • Meet as needed with pharmaceutical representatives to stay updated on new medications and to obtain samples for outpatient use
  • Responsible for checking or supervising the checking of medications stored on the inpatient unit of RBH at least monthly
  • Will procure necessary parenteral products
  • Responsible for the preparation of annual physical inventory of all articles used and contained in the pharmacy area or floor stock, crash cart & night locker of RBH
  • Maintains bookkeeping records in accordance with hospital policies
  • Responsible for seeing that all medications are stored safely and correctly and that all outdated medications are separate from usable medications and disposed of properly
  • Maintain records such as pharmacy files, patient profiles and charges, inventory narcotics and controlled substances per regulation, law and The Joint Commission standards
  • Responsible for developing constructive and cooperative working relationships with others
  • Provides assistance for doctors, secretaries and clients in getting patients signed up for Medication Assistance Programs (MAP), determining if a program exists, making suggestions for alternative medications as needed
  • Participate in quality improvement activities including committees, task groups, PIT teams, staffing and as assigned
  • Provides other duties as assigned

We are seeking pharmacist candidates with the following qualities:

  • Graduate of an accredited school of pharmacy with either Pharm.D or Bachelors in Pharmacy
  • Pharmacist must be licensed in Indiana and in good standing with the Indiana Board of Pharmacy
  • Pharmacist will complete a minimum of 30 continuing education units (CEU) in a 2 year period. A file is to be available to administration upon request.
  • Able to achieve the required competency or privileges at River Bend Hospital.
  • Valid driver's license, a driving record meeting Meridian's driving policy, reliable transportation, and proof of auto insurance required
