Mercy Health St. Vincent Medical Center is a faith based Catholic Healthcare Ministry committed to serving the poor and underserved of our community since 1853. Mercy Health St. Vincent Medical Center is a Regional Tertiary care facility serving a 150 mile radius of Northwest Ohio and Southeast Michigan. St. Vincent has been a well-established leader in quality healthcare as demonstrated by being verified as a Level I Adult and Level II Pediatric Trauma Center by the American College of Surgeons Committee on Trauma, designated as a STEMI Center, as well as Comprehensive Stroke Center verified by the Joint Commission. Mercy Health St. Vincent Medical Center has also partnered with Nationwide Children's Hospital of Columbus to create Nationwide Children's Hospital of Toledo on the campus of Mercy Health St. Vincent Medical Center.
Job Summary:
Manages patient transitions of care through the hospital and then to the appropriate post-acute setting. Assures third-party payers approval for any services provided. Assures appropriate documentation to support patient status, level of care and DRG.
Education:
Current RN license in the state of Ohio. BSN preferred, or must be obtained within 3 years.
Minimum Qualifications:
  • Must have a minimum of 3 years of clinical experience. Clinical specialty or case management certification preferred.
  • Case Management experience preferred. Case Management certification preferred and required within 3 years of hire date

Skills and Abilities:
  • Ability to analyze clinical, financial and social situations, identifying problems and alternative courses of action.
  • Willingness to be flexible, resourceful, and creative in problem solving.
  • Ability to act independently and offer suggestions and new ideas for improving performance and operations.
  • Ability to guide and direct the health care team to assure optimal outcomes. Clinical expertise in a specialty area.
  • Ability to organize and present information clearly and concisely.
  • Demonstrated interpersonal skills in order to effectively communicate with physicians, patients, families and other members of the interdisciplinary team.
  • Creative problem solving and critical thinking.
  • Excellent customer service skills.
  • Working knowledge of DRG assignment.
  • Current CPR certification required. ACLS and other certifications may be required.
