• Conduct thorough evaluations of patients in a home care setting, assessing their physical abilities and rehabilitation needs related to recovery from injury, disease, or surgery.
• Develop comprehensive treatment strategies using diverse modalities such as therapeutic exercises, manual therapy, and advanced rehabilitation equipment to alleviate pain and enhance mobility.
• Create and implement individualized treatment plans based on ongoing assessments and set clear, achievable therapeutic goals for each patient.
• Document all patient interactions, treatments, and progress accurately using electronic medical record (EMR) systems like Web PT, ensuring compliance with industry standards.
• Regularly reevaluate patient progress and adjust treatment plans proactively to ensure optimal recovery outcomes.
• Advise patients on the use of assistive devices, providing training to both patients and their caregivers to empower them in their recovery journey.
• Stay abreast of the latest research and best practices in physical therapy through continuous education and collaboration with professional networks.

Our diverse patient population includes individuals of all ages, from children recovering from sports injuries to elderly patients rehabilitating after surgeries or managing chronic conditions. You will work closely with each patient to create trust and provide compassionate care that meets their specific rehabilitation needs.

• Doctorate in Physical Therapy (DPT) from an accredited program.
• Active Physical Therapist license in New Jersey with a clean professional record, free from sanctions or board proceedings.
• Demonstrated proficiency in a variety of manual therapy techniques and evidence-based exercise prescriptions.
• Experience with electronic medical record systems, particularly EMR software like Web PT.
• Strong knowledge of up-to-date physical therapy methodologies supported by clinical research.

• A minimum of 3 years of hands-on experience in a physical therapy setting, particularly in outpatient or home care environments.
• Background in developing and implementing rehabilitation programs tailored to diverse patient needs.
