Job description

Position Summary 

This position works under the supervision of Manager B to answer calls from seniors, caregivers, professionals and other inquirers regarding programs and services. The position will perform in-depth screening of callers for in-home services utilizing procedures established by the Tennessee Commission on Aging and Disability (TCAD) and TennCare.

Pay Grade: 44

Salary Commensurate with Experience and Education

Minimum Qualifications

  • Two (2) years of work experience in case management, social work, education, or assisting with programs of referrals and client needs; AND 
  • Bachelor's degree from an accredited college or university in social services, nursing, sociology , or related field; OR   
  • An equivalent combination of related education and/or experience.   
  • Experience in case management programs assisting senior citizens/persons with disabilities preferred.  
  • Bi-lingual in Spanish and English preferred. 
  • PROOF OF EDUCATION, TRAINING, AND/OR EXPERIENCE IS REQUIRED. 

Duties and Responsibilities

  • Provides comprehensive information and referral services on community programs to consumer, caregivers, and professionals, as required, paying particular attention to all services offered through the Aging Commission of the Mid-South. 
  • Performs in-depth screening for consumers requesting in-home services to assist in determining which agency program could best meet the consumer's needs. 
  • Completes data-entry of consumer and call information into software database required by TCAD or TennCare. 
  • Initiates contact with service provider agencies on the consumer's behalf when appropriate; makes follow-up contacts to determine consumer's needs are met and whether any additional referrals are necessary and/or interventions are needed. 
  • Cross-trains other Information and Assistance staff to provide services to clients during all regular business hours. 
  • Establishes and maintains a cooperative relationship with all local information and assistance programs, referring agencies and other social service organizations. 
  • Records and analyzes caller data to be used for planning and development of future services. 
  • Attends community social service functions for educational enrichment and outreach opportunities. 
  • Attends professional training programs, workshops and staff meetings as required. 
  • Performs other related duties as assigned or directed.

  • Ability to establish and maintain effective working relationships with clients, agencies, and co-workers. 
  • Knowledge of modern office practices, procedures, and equipment; skilled at operating office equipment. 
  • Ability to become proficient in Microsoft Word and Excel. Familiarity with computerized databases. 
  • Ability to plan, organize, and maintain an even flow of work. 
  • Ability to provide effective customer service in a fast-paced environment. 

Special Requirements 

  • Must possess a valid Tennessee driver's license or secure one upon date of employment. 
  • Must have daily access to an operable automobile and meet County standards for automobile insurance upon date of employment. 
  • Must seek Alliance of Information and Referral Systems certification in Aging & Disability Services after one year of full-time employment. 
  • Must successfully pass a background check to include: Tennessee Abuse Registry, 
  • Tennessee Felony Offender Registry, National and Tennessee Sexual Offender Registry.

Shelby County Resident Disclaimer

All employees hired after September 1, 1986, must be residents of Shelby County and shall continue to reside in the County as a condition of their employment.  The residency requirement shall not apply to certain public safety/civil service employees.  This means exemption from this policy is in effect for the following departments and positions: (1) Sheriff Deputy Patrol Officers, Deputy Jailers and Dispatchers, employed by the Sheriff Department, (2) Correction Officers employed by the Division of Corrections, (3) Firefighters, Paramedics and Dispatchers employed by the Shelby County Fire Department.  The residency exemption for public safety/civil service employees does not include Appointed positions with Sheriff's Office, Division of Corrections or the Fire Department.Â
