Physical Therapist Assistant

Optimal Home Care is looking for a Physical Therapist Assistant to provide services for our patients!

Position & Responsibilities:

  • Under the supervision of the Physical Therapist, assists in evaluation and development of the rehabilitative plan of care.
  • Practices and maintains patient safety and dignity by abiding by Federal, State, Local and Optimal policy & procedures as pertains to patient protected health information and HIPAA (Health Information Portability and Accountability Act).
  • As directed, routine strengthening and therapeutic exercises are performed.
  • Assists in teaching exercise programs to patients and family.
  • Assist patients in use of self-help devices.
  • Consults with the therapist regarding the outcome of home visits and report physical findings in written form.
  • Maintains daily progress notes on patients and submits same with at least seven (7) days of the visit.
  • Participates in therapy and other in-service programs.
  • Participate in staff and agency development activities, including orientation for new employees.
  • Attends rehabilitation department staff meetings.
  • Performs related and other duties as required.
  • Is supervised at least once monthly by the Physical Therapist.
  • Complies with agency Policies and Procedures as well as Employee Handbook detailing Human Resources policies.
  • Other duties / responsibilities that support the agency's core values of Commitment, Advocacy, Respect, and Excellence.
  • Completes emergency preparedness training as identified as appropriate for this employee level.
  • Participates in exercises and drills for emergency preparedness, as required.
  • Other duties and tasks as assigned by the Optimal Home Care Emergency Command Center communication tree in the event of a local, regional or national emergency and/or disaster.

Requirements:

  • Preferably One (1) year of experience.
  • Graduated from a physical therapist assistant curriculum approved by the Commission on Accreditation in Physical Therapy Education of the American Physical Therapy Association or has passed a national examination for physical therapist assistants.
  • Must be currently licensed and in good standing in the state of Colorado.
  • Must be able to communicate in the language of the client and document client related activity as appropriate.
  • Possesses adequate and effective written and verbal communication skills.
  • Possess adequate and effective interpersonal skills.
