Resident Experience Coordinator with CNA certification - Full-Time
Santa Rosa Post Acute

Santa Rosa Post Acute is seeking a dedicated and resident-focused Resident Experience Coordinator with Certified Nursing Assistant (CNA) certification required, to join our team. We are committed to providing exceptional care while fostering a supportive and collaborative work environment for our staff.

Schedule
  • Full-Time
  • 11:00 AM - 7:30 PM
  • Schedule/rotation to be determined
Position Responsibilities

In addition to providing excellent resident care, this role will focus on enhancing the admission experience and improving resident satisfaction through:

  • Assisting residents with activities of daily living while maintaining dignity and respect
  • Passing meal trays and assisting residents during meal service
  • Participating in change-of-shift rounds to help reduce fall risk and improve resident safety
  • Conducting visits with all new admissions on Days 1, 2, and 3 following admission
  • Making follow-up calls to responsible parties (RPs) on Days 1, 3, and 7 after admission
  • Completing Angel Rounds documentation for assigned residents
  • Identifying resident needs, concerns, and opportunities for improvement
  • Communicating findings daily to Administration, Admissions, DON, DSD, and other department leaders as appropriate
  • Presenting resident concerns, trends, and outcomes during monthly QAPI meetings
  • Collaborating with licensed nurses and the interdisciplinary team to promote quality care and positive resident outcomes
  • Maintaining accurate and timely documentation
Qualifications
  • Active California Certified Nursing Assistant (CNA) certification required
  • Passion for serving seniors and enhancing the resident experience
  • Strong communication and customer service skills
  • Reliable, organized, and team-oriented
  • Ability to work effectively in a fast-paced healthcare environment
