Position Overview

Thrive Collective Health Center is on the lookout for a committed and experienced Licensed Clinical Social Worker (LCSW) to join our team in a hybrid work model. Work Environment

This position operates in a hybrid work environment, combining remote and in-person interactions to maximize flexibility and client accessibility. LCSWs are expected to conduct individual therapy sessions via video conferencing and in-person appointments at our clinic as needed. Key Responsibilities
  • Conduct thorough assessments of clients' mental health needs through clinical interviews and diagnostic tools.
  • Design and implement personalized treatment plans that adhere to best practices in social work.
  • Engage in individual counseling sessions, providing evidence-based interventions tailored to clients' unique circumstances.
  • Maintain accurate client records in compliance with all state and federal regulations, utilizing our electronic health record (EHR) system.
  • Collaborate with healthcare professionals, including psychiatrists, nurses, and case managers, to facilitate comprehensive care for clients.
  • Advocate for clients by connecting them with community resources and support services that enhance their pathway to mental wellness.
  • Participate in regular team meetings and training sessions to share insights and strategies for improved client outcomes.
Patient Population / Client Focus

As part of our community health clinic, you will serve a diverse clientele, including individuals facing various challenges such as anxiety, depression, trauma, and relationship issues. Our commitment to serving marginalized populations means you'll often work with clients from varying socio-economic backgrounds, cultures, and life experiences. Understanding the unique needs of these individuals is crucial in creating accessible and effective treatment plans.

Qualifications
  • A Master's degree in Social Work from an accredited institution.
  • Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of Oregon with no history of disciplinary action.
  • Demonstrated proficiency in clinical social work practices, including a strong understanding of mental health diagnosis, treatment planning, and therapeutic modalities.
  • Exceptional interpersonal skills with a proven ability to establish rapport with individuals from diverse backgrounds.
  • Familiarity with telehealth technology and computer systems for client management.
Preferred Experience
  • Two or more years of experience providing clinical social work services in a community health or similar setting.
  • Experience working with specific populations, such as children, adolescents, or individuals with substance use disorders.

Compensation and Benefits

We offer a competitive annual salary ranging from USD 115,000 to 130,000, commensurate with experience and specialized skills.
