About Us:
Tree Of Life, a Collage Rehabilitation Partner, is a nationally recognized leader in brain injury rehabilitation. For over 35 years, we've specialized in helping clients with moderate to severe traumatic and acquired brain injuries. Our programs address post-concussion syndrome, stroke, memory loss, spinal cord injuries, neurobehavioral challenges and head injuries from accidents or sports.
Our individualized treatment plans are creative, effective and designed to meet the complex needs of our clients. Our interdisciplinary teams work collaboratively with our medical directors to guide each client on their journey to recovery.
Position Overview:
Tree of Life is seeking a dedicated Physical Therapist to provide group and individual physical therapy services to clients in collaboration with an interdisciplinary treatment team to include psychology, physical, speech, recreational and occupational therapies. Specific areas of treatment may include, but are not limited to: vestibular issues, pain management, cervicogenic issues, flexibility and/or strength deficits, gait and balance disorders, functional mobility and transfers, mobility equipment assessment and recommendations and development of home exercise programs.
The ideal candidate is knowledgeable, kind, confident, positive, patient, and adaptable. They excel in creating a supportive and encouraging environment, fostering trust and collaboration with clients, families and interdisciplinary teams. They should also be able to multi-task and communicate effectively tailoring treatments to each individual's unique needs, ensuring personalized and effective care.
Responsibilities:
• Review patient medical histories and diagnose conditions through observation and patient discussions
• Develop and implement individualized treatment plans with clear goals
• Utilize exercises, stretching, equipment, and hands-on therapy to manage pain and improve mobility
• Document patient progress and adjust treatment plans as needed
• Educate patients and family members about the recovery process and provide ongoing training and support for discharge planning
Requirements:
• Bachelor's degree in Physical Therapy
• Valid PT license required
• Master's or Doctorate in Physical Therapy preferred
• Experience in treating neurologically related physical disorders in community based settings is strongly preferred
• Valid driver's license and a clean driving record
