Job Summary and Qualifications
Schedule: Part Time - 0.7 FTE or 28 hours minimum. Rotating Days/Evenings + Weekends
Licensure: Current California state Pharmacist License required
Hospital/Acute care experience strongly preferred
As the Centralized Order Entry Pharmacist, you will ensure that medication orders meet standards for patient care. In this role, you will monitor prescriptions, verify that the dosage and type of medication are appropriate for the patient's condition, enter the orders into an online database, track and oversee prescriptions, evaluate patient care plans, and provide any necessary recommendations for the patient.
What you will do in this role:

  • Reviews and interprets all physician orders received, using the patient profile
  • Screens for drug interactions, allergies, order duplications, appropriate diagnosis, renal and liver function prior to order entry
  • Monitors for incompatibilities, concentration and rate on intravenous drugs
  • Prepares written or oral report at change of shift to communicate incomplete assignments and potential patient safety concerns to monitor in order to ensure continuity of medication therapy.
  • Ensures facility procedures are followed when non-formulary drugs, target drugs, restricted drugs and investigational drugs are ordered
  • Investigates and reports adverse drug events and medication incidents
  • Facilitates dissemination of drug information in a timely manner to achieve high quality, cost effective, positive therapeutic outcomes when consulted
What qualifications you will need:
  • Bachelor's degree from an accredited college of pharmacy is required.
  • Pharm D is preferred
  • 1+ year hospital experience is preferred.
  • Meditech experience strongly preferred
  • Current pharmacy license in appropriate state required.
