Job Description:
The Hospice/Outpatient Bereavement Coordinator provides support to patients and family members through the final stages of illness by coordinating hospice services. This role includes offering services in various settings including private homes, skilled nursing facilities, and others, focusing on providing comprehensive bereavement support.
Responsibilities:
  • Coordinate hospice and bereavement services for patients and families.
  • Provide pain and symptom management, medical social services, counseling, spiritual support, and other hospice services.
Requirements:
  • Bachelor's degree in social services, counseling, or psychology-related area required; Master's degree preferred.
  • Healthcare Provider Level BLS required.
  • Experience or education in grief or loss counseling required.
  • Valid driver's license and auto insurance as required by law; personal automobile use required.
  • Recent experience in hospice preferred; additional experience in a medical setting preferred.
