Job Summary:

Position Title: CNA / QMAP CNA
Location: Craig, CO 81625
Duration: 48 weeks
Work Schedule: Night Shift - Awake Overnight
Shift Options: 8s, 10s, and 16s may be available based on facility needs
Hours: 40 hours per week
Pay rate: $35/hr
Responsibilities for this position include, but are not limited to:
  • Provide compassionate, direct care and support to residents.
  • Assist residents with activities of daily living, including bathing, dressing, grooming, toileting, and mobility.
  • Monitor and report changes in residents' condition, behavior, or overall well-being.
  • Take and document vital signs and other required observations.
  • Assist with transfers, positioning, ambulation, and other mobility needs.
  • Support residents with meals, hydration, and nutritional needs.
  • Follow individualized care plans and facility policies.
  • Administer medications and perform medication-related duties within the scope of QMAP certification and facility requirements.
  • Maintain accurate and timely documentation.
  • Communicate effectively with nurses, healthcare providers, supervisors, residents, and families as appropriate.
  • Maintain a clean, safe, and supportive living environment.
  • Respond appropriately to resident needs and emergencies.
  • Perform other duties as assigned by facility leadership.
To be considered for this position, you should have: [Skills, Education, or Experience]
  • Current CNA certification appropriate for practice in Colorado.
  • QMAP certification is required prior to the start date.
  • Candidates may be submitted without QMAP certification, provided they are able to obtain the certification before starting.
  • Previous CNA experience in a skilled nursing, group home, residential care, or similar healthcare setting is preferred.
  • Strong communication, documentation, and interpersonal skills.
  • Ability to work independently during awake overnight shifts.
  • Ability to work the required Monday-Thursday, 7:00 PM-7:00 AM schedule
