Certified Nurse Assistant/QMAP- $4,000 Sign on Bonus Available

Schedule: Monday-Friday, 8:00a-5:00p

The CNA/QMAP supports clinic operations by providing direct participant care, medication administration (within scope as a Qualified Medication Administration Person), and assisting the clinic team to ensure safe, efficient, and high-quality care. This role combines hands-on clinical support with medication administration and patient flow coordination in a clinic setting.

What is PACE?

PACE or Program of All-inclusive Care for the Elderly is a federal and state funded program that provides comprehensive, coordinated, and proactive whole-person care to help maintain and improve seniors' health so they can live safely in their homes. There are 180 PACE programs, serving more than 80,700 participants in 33 states.

The number of people aged 55+ is increasing in Grand Junction, CO and the demand for eldercare professionals is on the rise.

You Bring:

• Minimum of 2 years’ experience as a CNA required.

• Minimum of 2 years’ experience as a QMAP required.

• Previous clinic, outpatient, or long-term care experience preferred.

• Current Certified Nursing Assistant (CNA) certification in good standing in the state of Colorado required.

• Current QMAP certification in the State of Colorado required.

• CPR Certification is maintained throughout employment.

• Knowledge of basic nursing care and infection control practices

• Understanding of medication administration protocols within QMAP scope

• Strong attention to detail and documentation accuracy

• Effective communication and interpersonal skills

• Ability to multitask and work in a fast-paced clinic environment

• Basic computer skills and familiarity with EHR systems
