Full Job Description:
Job Summary:
We're seeking a compassionate and detail-oriented Registered Nurse to lead preventive care efforts through Medicare Annual Wellness Visits. This role focuses on improving patient outcomes by conducting assessments, identifying health risks, and developing individualized wellness plans for eligible Medicare beneficiaries.
Key Responsibilities:
  • Conduct subsequent Medicare Annual Wellness Visits in compliance with CMS guidelines
  • Perform health risk assessments, cognitive evaluations, and medication reconciliations
  • Collect and document patient health histories, vital signs, and functional assessments
  • Collaborate with PCP to develop personalized wellness plans focused on disease prevention and health promotion
  • Educate patients on recommended screenings, immunizations, and lifestyle modifications
  • Collaborate with physicians and interdisciplinary care teams to coordinate follow-up services
  • Maintain accurate, timely documentation in the electronic health record (EHR)
  • Uphold quality standards and regulatory requirements for Medicare programs
  • Schedule Appointments
  • Travel to alternative NBMC locations to perform Medicare Wellness Visits.
  • Administer Immunizations
  • Queue orders for provider approval.
  • Other Duties as Assigned
Qualifications:
  • Active Oregon RN license
  • Minimum of 2 years clinical experience
  • Strong knowledge of Medicare coverage and CMS guidelines for wellness visits
  • Excellent communication, critical thinking and interpersonal skills
  • Proficiency with documentation protocols.
