Job description

Citizens Memorial Healthcare Foundation (CMH)in Bolivar, MOis hiring a Social Services Designee to care for the residents at Community Springs Healthcare Facility! CMH is made up of 34 primary and specialty clinics, 6 Long-Term Care facilities and 1 residential care facility. Offering acute care, emergency services, ambulatory care, pharmacies, walk-in clinics, surgeries, addiction recovery, therapies and more, CMH has its community covered!  

About the area: El Dorado Springs is a progressive, full-service city in Southwest Missouri, known for its rich history and family-friendly atmosphere. It's the largest city in Cedar County and was incorporated in 1881, centered around a spring believed to have healing properties. The city's location between Kansas City, Springfield, and Joplin provides easy access to various amenities, including the Wayside Inn Museum, the Opera House Cinema, and El Dorado Springs City Park. 

About the facility: Community Springs Healthcare Facility (CSHCF) is a 105-bed long-term care skilled nursing facility providing transitional or long-term care. A 15-bed high-functioning memory care unit provides specialized care for residents with Alzheimer's or other memory issues. 

A typical day includes: As a Social Services Designee, you will assist with admission/discharge planning. You will complete regulatory paperwork, assist with family/resident concerns/needs. The Social Services Designee also coordinates the transportation of residents to appointments. You will work to utilize the communities’ resources to assist in the discharge of short stay residents.  

  • Delivering resident mail the same day as received 

  • Assessing each resident’s psychosocial status  

  • Serving as a liaison between the families of residents and the health care team in time of crisis 

  • Making arrangements for obtaining needed adaptive equipment, clothing, and personal items 

Job Requirements: 

  • Education:High school diploma, MHCA Designee Training required 

  • Experience: 1 year of long term care/healthcare preferred 

  • Skills: Collaborative, detail oriented, problem-solving 

  • Licensure/Certification:Family and Friends CPR within 6 months of employment  

  • Physical: Ability to stand for 10% of the day and sit for the remaining 90%
