GENERAL SUMMARY: 

The Inpatient Clinical Documentation Integrity (IP CDI) Supervisor uses clinical, coding, and CDI knowledge and experience to improve the quality and integrity of documentation in the electronic medical record (EMR). Identifies factors influencing the complexity of a patient’s diagnosis and treatment plan. Serves as a CDI resource and works collaboratively with physicians, residents, advanced practice providers, nursing, and ancillary services. Champions for the advancement of the Henry Ford Health IP CDI program. 

PRINCIPLE DUTIES AND RESPONSIBILITIES: 

1. Oversee day-to-day CDI frontline team operations: personnel scheduling, retrospective review assignment, work queue (WQ) troubleshooting and clean up, query resolution and reconciliation, additional daily responsibilities as determined 

2. Review clinical evidence and diagnostic results in the electronic medical record (paper record during downtime) to ensure that documentation is complete and compliant, accurately reflecting complexity of care and resource consumption 

3. Use clinical experience and knowledge of ICD-10-CM Coding Guidelines and risk adjustment measures to identify opportunities to clarify documentation 

4. Identify and bring forward opportunities for education 

5. Conduct CDI quality audits for completeness of review and opportunity capture. Communicate findings and opportunities for education/remediation to operational managers. 

6. Conduct CDI quality audits of queries for compliance with ICD-10-Coding Guidelines, regulatory standards, and industry best practices. Communicate findings and opportunities for education/remediation to managers. 

7. Formulate compliant queries and adhere to approved process for query resolution 

8. Collaborate with coding on retrospective reviews and queries 

9. Perform reconciliation reviews to ensure query impact is accurately captured 

10. Review CDI/Coding DRG mismatches and follow approved process for resolution 

11. Conduct subsequent case reviews and identify new query opportunities 

12. Collaborate with Quality department to ensure that documentation supports quality pathways and initiatives 

13. Understand and value the importance of data quality and integrity 

14. Strive to establish and strengthen interdepartmental relationships and workflows 

15. Maintain knowledge of new and existing ICD-10-CM Coding Guidelines, MS and APR-DRG reimbursement, and documentation requirements 16. Answer clinician questions regarding level of documentation detail for accurate, complete coding of conditions and procedures 

17. Serve as a resource for clinician education regarding the significance of appropriate documentation to support acuity of care 

18. Serve as a resource for coding staff with clinically based questions related to assignment of ICD-10-CM codes 

19. Assess workflows, forms, policies, and processes regularly and recommend changes as appropriate

EDUCATION/EXPERIENCE REQUIRED:

• Registered Nurse (RN), OR • Certified Nurse Practitioner (CNP), OR • Physician Assistant (PA-C), OR • ECFMG Certified International Medical Graduate • Demonstrated clinical competence as evidenced by five years’ experience in an acute care setting 

• Minimum three years IP CDI experience 

• Demonstrated knowledge of ICD-10-CM Coding Guidelines • Knowledge of revenue cycle processes preferred.

 CERTIFICATIONS/LICENSURES REQUIRED: 

• Current Registered Nurse license, OR • Current Certified Nurse Practitioner (CNP) or Physician Assistant (PA-C) license, OR • Current Educational Commission for Foreign Medical Graduates (ECFMG) credentials • Certified Clinical Documentation Specialist (CCDS) preferred
