Job Requirements
POSITION SUMMARY
The Clinical Pharmacy Supervisor provides practice leadership, coordination and oversight of clinical pharmacy services at Health First's Holmes Regional Medical Center (HRMC). The Clinical Pharmacy Supervisor directly supervises the HRMC Clinical Pharmacist team while ensuring safe, effective, evidence-based and cost-conscious medication use. The Clinical Pharmacy Supervisor maintains a blend of clinical practice, leadership, medication stewardship, clinical governance, education and program development responsibilities designed to advance pharmacy practice at HRMC. The Clinical Pharmacy Supervisor guides pharmacist workflow supports interdisciplinary collaboration, and partners with pharmacy leadership to advance departmental and organizational goals.
In partnership with the Director of Pharmacy Tertiary Hospital and the Medication Safety Coordinator, the Clinical Pharmacy Supervisor serves as a co-chair of the Hospital Division Pharmacy and Therapeutics Committee (P&T). The Clinical Pharmacy Supervisor advances medication safety, clinical governance, regulatory compliance, clinical program development, associate education, performance improvement, and standardized pharmacy practice across assigned clinical practice areas.
PRIMARY ACCOUNTABILITIES
  1. Provides direct clinical leadership, supervision, and operational support to the Clinical Pharmacist team, including assignment of work, prioritization of clinical activities, workflow coordination, performance management, competency assessment, professional development, project oversight, preceptor development, and escalation support for daily pharmacy practice needs.
  2. Promotes evidence-based pharmacotherapy, medication regimen review, therapeutic monitoring and dosing, antimicrobial stewardship, anticoagulation management, transitions of care, and other clinical programs as assigned.
  3. Serves as a clinical resource to pharmacists, physicians, nurses, advanced practice providers, and interdisciplinary teams regarding medication therapy, formulary management, drug information, and medication safety concerns.
  4. Identifies, develops, implements, monitors, evaluates and continuously improves clinical pharmacy services, protocols, guidelines, order sets, therapeutic interchange processes, and medication-use policies to improve patient outcomes, patient safety, operational efficiency and organizational performance.
  5. Serves as co-chair of the Hospital Division P&T Committee in collaboration with the Director, Pharmacy Tertiary Hospital and Medication Safety Coordinator.
  6. Leads and supports P&T including agenda development, formulary review coordination, clinical content oversight, medication-use evaluations, adverse drug event and medication safety review, policy/procedure governance, follow-up on committee decisions, and implementation of approved medication-use initiatives.
  7. Supports antimicrobial stewardship, medication safety, regulatory readiness and compliance with applicable federal, state, and local laws, accreditation requirements, Board of Pharmacy rules, organizational policies, and professional standards.
  8. Monitors clinical pharmacy performance measures, intervention documentation, service coverage, productivity, quality outcomes, associate development goals, and other key metrics. Identifies trends and implements action plans supporting service improvement, professional growth and organizational objectives.
  9. Partners with pharmacy leadership to support staffing plans, scheduling needs, service coverage, issue resolution, implementation of departmental priorities, formulary compliance, medication stewardship initiatives and cost-effective medication use.
  10. Collaborates with pharmacy informatics, providers, nursing, and operational leaders to improve electronic health record workflows, clinical decision support, medication order sets, and pharmacy-related technology processes.
  11. Maintains clinical competency and operational awareness through recurrent direct patient care responsibilities, including interdisciplinary rounding, service line support, clinical consultation, medication therapy management, resident and student precepting, and other clinical pharmacy activities based on departmental needs. Clinical practice activities support professional competency, interdisciplinary collaboration, resident education, and advancement of clinical pharmacy services.

Work Experience
MINIMUM QUALIFICATIONS
  • Education: Bachelor's degree in pharmacy (BPharm).
  • Work Experience: Any one of the following:
  • Five (5) years' hospital pharmacy experience OR
  • Three (3) years' hospital pharmacy experience as well as completion of post-graduate year one (1) residency OR
  • Completion of post-graduate years' one (1) and two (2) residencies.
  • Licensure: Pharmacist licensure in the State of Florida.
  • Certification:
  • Any one of the following prior to start date and maintained:
  • American Heart Association Basic Life Support (AHA BLS) Healthcare Provider Completion Card OR
  • American Heart Association Advanced Cardiac Life Support (AHA ACLS) AND Pediatric Advanced Life Support (AHA PALS) Healthcare Provider Completion Cards.
  • Board of Pharmacy Specialists certification within one (1) year of start date and maintained.
  • Skills/Knowledge/Abilities:
  • Maintains professional competency and awareness of current clinical pharmacy practice trends, literature, regulatory standards, and health-system pharmacy best practices.
  • Knowledge of pharmacotherapy, medication safety, formulary management, clinical governance, regulatory requirements and hospital pharmacy workflow.
  • Demonstrated communication, collaboration, problem solving, prioritization, coaching and decision making skills.
  • Proficiency with electronic medical record systems, pharmacy technology, reporting tools, and standard office software.

PREFERRED QUALIFICATIONS
  • Education:
  • Doctorate's degree in Pharmacy (PharmD).
  • Completion of an ASHP-accredited PGY1 residency and PGY2 residency, or equivalent advanced practice experience
  • Work Experience: Five (5) years' hospital pharmacy experience, prior formal or informal leadership experience, experience supporting clinical pharmacy services, medication safety, antimicrobial stewardship, oncology, critical care, pediatrics, transitions of care, or other specialty practice areas.
  • Certification: Board of Pharmacy Specialists certification
  • Skills/Knowledge/Abilities: Experience with Epic or other electronic medical record systems, clinical decision support, order set optimization, quality dashboards, Medication Use Evaluations, quality improvement initiatives, and pharmacy practice standardization across hospital sites.
  • Demonstrated ability to develop, implement, evaluate and sustain clinical pharmacy services and service line initiatives.
  • Experience supporting Pharmacy and Therapeutics Committee activities, formulary management, medication use policy development, medication stewardship initiatives and multidisciplinary clinical programs.
  • Experience precepting pharmacy students and residents or supporting pharmacy residency programs.
  • Experience leading or participating in quality improvement initiatives, clinical program development, performance improvement projects or pharmacy practice standardization.

PHYSICAL REQUIREMENTS
