As a PT in this role, you will become an integral teammate in our innovative Take Your Therapist Home Program!
What is Take Your Therapist Home?
In a world of shorter and shorter SNF stays, Take Your Therapist Home (TYTH) is a one-of-a-kind program where you as the clinician work directly with patients in the SNF and home health providing a continuity of care allowing for you to have a greater impact on patient outcomes!
You will be primarily in the home health setting with dedicated time and caseload support for the SNF. You will have a localized coverage area in the surrounding area of the SNF. No more ever-expanding home care coverage areas. Your schedule is based on a traditional work week.

This role requires you to travel locally to our facilities located in Grand Rapids, Ionia, Leonard, Muskegon, Whitehall, Wyoming.
Schedule: Full Time - Monday- Friday
General Summary:
In this role you will provide physical therapy services to patients impaired by physical dysfunction, including assessment, treatment planning and implementation of treatment plans. Participates in patient care conferences, program development, and screenings as appropriate. Provides supervision for PTAs and aides as assigned. Completes appropriate documentation and adheres to policies, procedures and current standards of clinical practice.
Your Responsibilities:
  • Conduct physical therapy screenings and evaluations to determine patient physical status.
  • Provide physical therapy treatment consistent with the professional and ethical standards of the APTA for Physical Therapists.
  • Develops and implements treatment plans using strengthening, modalities and retraining to reach patientsโ€™ highest potential for independence.
  • Provides direct patient care in accordance with productivity standards by use of graded activities to facilitate the restoration of physical and functional abilities.
  • Checks, instructs, assigns and reviews the work of PTAs and aides as assigned.
  • Provides documentation to maintain quality of the medical record as it relates to accuracy, continuity of care and reimbursement guidelines.
  • Participates, as requested, in patient care conferences, per company guidelines.
  • Participates in CQI activities as required ensuring continuous quality of care.
  • Promotes current standards of clinical practice through participation in continuing education and in-service programs.
  • Coordinates and/or participates in in-service programs for internal/external groups.
  • Participates in new program development.
  • Participates in company meetings and additional activities directed to facilitate effective departmental operations.
Required Qualifications:
  • Successful completion of an accredited Physical Therapy program
  • Licensure as required by the State of Michigan.
  • Valid Driver's License
Skills and Abilities:
  • Ability to exercise professional judgement and initiative when analyzing problems and recommending solutions.
  • Initiative and flexibility in the work environment.
  • Ability to react effectively to the needs of patients and their families.
  • Physical Ability to bend, position, transfer and lift patients weighing from 80 pounds to over 300 pounds, without restriction, utilizing lifting devices clinically recommended for the patient.
